L CALL MEETING TO ORDER:
The Employee Safety Committee meeting was called to order at 9:04 a.m. in the Upper
Level Conference Room with the following representatives

lI. APPROVAL OF MINUTES:
Motion by Susan Schwartz, seconded by Mark Stevens, to approve the minutes of the
Employee Safety Committee meeting of July 19, 2001 as submitted. VOICE VOTE:
Motion carried. Minutes are to be dated and posted in respective departments.
Assistant Village Manager Crotty introduced David Zima who will be the Committee
representative for the Administration, Finance and Engineering Departments and Michael Janney
a representative for the Fire Department. He indicated that the Committee meets on the third
Thursday of each month.
111. DISCUSSION OF INCIDENTS
Representative Krob explained the incident of a Golf Club Cafe Manager who burned the
palm of her hand when removing a pot from the stove. He noted that she did not use a
potholder nor did she seek medical treatment. Assistant Village Manager Crotty
explained the correct forms to be completed noting that when no medical treatment is
necessary, an "Incident/First Aid Report"should be submitted. When medical treatment
is necessary, a "Form 45: Employers First Report of Injury or Illness" as well as a
"Supervisor's Investigation Report" should be submitted. One form or the other
depending on whether medical treatment was necessary. He also noted that if medical
treatment was necessary at a later date after submitting the Incident/First Aid Report, a
Form 45 and Supervisor's Investigation Report needs to be submitted.

Assistant Village Manager Crotty further explained that when an incident incurs a cost of
$100 or more, a determination of "avoidable" or "unavoidable" is made by the
Department Head, Supervisor, and Safety Committee representative. A simple majority
rules the determination. The employee can formally appeal any avoidable determination
to the Executive Safety Committee or an informal internal appeal to the Assistant Village
Manager in certain instances. He noted that prior to his representation as the Safety
Committee Coordinator, the Committee made the avoidable/unavoidable determination.
Most communities have their Committees making the determination, but our Committee
discusses each incident noting preventative measures.
The incident of a cut left index finger tip of a Golf Club Cafe Cook was discussed.
Assistant Village Manager Crotty indicated that since this employee as well as the
previously discussed employee were seasonal, part-time employees, no formal
determination is made on the incidents; but all incidents are discussed at the Committee
meetings. He also noted that if a call is made for medical aid, the employee should seek
treatment.
IV. DEPARTMENTAL SAFETY INSPECTION REPORTS: None.

VI. DISCUSSION OF SAFETY SUGGESTIONS: None.
VII. GENERAL DISCUSSION:
Assistant Village Manager Crotty explained IRMA to the new Committee members
noting that IRMA is a third-party administrator who processes general liability and
workers compensation claims and defends the Village. IRMA stresses safety and
reduction in claims with programs such as the recent "Strive for 25" publishing the
Claim-O-Meter in the Quarterly Membership Loss Report showing a performance gauge.
He explained the Report noting the analysis of the Village's claims frequency and claims
experience as well as claims listed by department. He noted that IRMA groups
departments and includes other departments along with the Public Works department.
Assistant Village Manager Crotty also distributes a quarterly report he has created
showing in more detail the frequency of claims by our departments.
Assistant Village Manager Crotty noted that objective suggestions are past due and again
requested Committee members to submit their lists within the next two weeks. He
reminded them to review the IRMA report as well as his report and look for types of
injuries and patterns to determine areas were training is needed based on department
trends. He informed Committee members that the Executive Committee has also been
requested to submit objective suggestions.
Assistant Village Manager Crotty informed Committee members of IRMA's program to
be introduced next year, "Drive for 25", striving to reduce vehicle accidents over the next

three years. IRMA has hired a new training director and is working on improving their
training. Through the years, the Public Works and Police departments have participated
in IRMA's NAPD training, and now it is also available to the Fire Department.
Assistant Village Manager Crotty indicated that he will be checking to see if we should
be looking at specialized training, and Public Works representative Stevens noted that the
Snow Rodeo presented good competition. Police representative Nemec indicated that
pursuit driving training is good. Fire representative Naus questioned setting up our own
program. Assistant Village Manager Crotty indicated that we have the annual Safety
Checkride Program, and the Public Works and Fire departments have employees certified
in training. Police representative Nemec will check to obtain certification as a trainer.

Motion by Gary Naus, seconded by Norm Nemec,to adjourn the meeting at 10:15 a.m.
