TO USE TOLL HIGHNRr RIGHT-08-HAY
BE IT RESOLVED BY THE PRESIDENT AND BOARD OF TRUSTEES OF THE VILLAGE OF
OAK BROOK, DU PAGE AND COOK COUNTIES, ILLINOIS that the Village .President and
Village Clerk are hereby authorized and directed to execute an application to
the Illinois State Toll Highway Authority for a permit to use toll highway
right-of-way for the installation, operation and maintenance of an eight-inch
(811) ductile iron water main all as described in the application therefor, a
copy of which is attached hereto as Exhibit A. The form of application
attached hereto is hereby approved.

President and Board of Trustees of the Village of Oak Brook, DuPage and Cook
Counties, Illinois, according to a roll call vote as follows:

(1) The applicant hereby applies to The Illinois State Toll Highway Authority (hereinafter called
"Authority" to grant a concession consisting of a permit to: (Briefly describe proposed use of Toll
Highway Right of Way).
Construct and maintain an 8" ductile iron watermain in
polyethelene encasement in the west right-of-way of the
Tri-State Tollway between Salt Creek and 31st Street.

(3) The Applicant attaches hereto and makes a part hereof plans and specifications of structure which
use Toll Highway flight of Way. Applicant agrees to furnish such additional plans and specifications of
structure as the Authority may request. In the event a permit is issued, applicant agrees to construct such
work in accord with the plans and specifications submitted to and approved y the Authority.
w. (4) The applicant further agrees and understands that the concession in the form of a permit herein
requested, if issued, does not sell, lease, create an easement, or otherwise dispose of or encumber the Toll
Highway Right of Way, or any part thereof, and does not create any charge or lien on the revenues of the
T o Highway.
(5) 'rhe applicant further agrees that, in the event a permit is granted, applicant will before
commencing work under the permit, comply with all the Utility Policy' & Regulations as adopted by the
Authority on August 31 1972 as amended and the acceptance,notice,bond and insurance requirements of
the Authority,as set forth in the permit.

CALL TO ORDER
1. MEETING CALL:
The Regular Meeting of the Board of Trustees was called to order by
President Bushy in the Samuel E. Dean Board Room of the Village Commons
at 7:30 p.m. Pledge of Allegiance was given.
2. ROLL CALL: ROLL CALL
The Deputy Clerk called the roll with the following persons

Trustee Skinner moved to approve the minutes of the Regular Meeting of
June 23, 1992 as amended and waive the full reading thereof. Seconded
by Trustee Payovich. VOICE VOTE: Notion carried.
05. CONSENT AGENDA:
Trustee Skinner requested Item 5.D., "An Ordinance Amending Article XIV
of the Code of Ordinances of the Village of Oak Brcok Relating to
Village Recreational Facilities (Age Restrictions for Junior Membership
and Unsupervised Swimming - Sports Core)", be removed from the Consent
Agenda for discussion.
Trustee Skinner moved to approve the Consent Agenda. Seconded by
Trustee Payovich. ROLL CALL VOTE:
Ayes: 5-Trustees Bartecki, Payovich, Shumate, Skinner and Winters.
