Our office has reviewed the Motor Fuel Tax File for the contract for
Section 99- 00031- 00 -FP. It is necessary for the Village to pass a supplemental
resolution in the amount of $140,536.42 to enable our office to authorize the
expenditure of this amount and close this section. All of the other required
paperwork has been submitted

RECOINENIENDATION: That the Village Board approve the "Supplemental Resolution for
Improvement by Municipality Under the Illinois Highway Code ".
At your meeting of December 14, 1999, you approved a "Resolution for Improvement by Municipality
Under the Illinois Highway Code" in the amount of $500,000 for the MFT portion of the 2000 Paving
Project. During construction, two change orders were approved which increased the contract amount
from $557,531.67 to $642,149.51. Since that time, final quantities have been calculated and the final
contract price is $640,536.42.
Since this project is paid from MFT funds administered by IDOT, we must follow IDOT procedures.
The attached letter dated December 3, 2001 from Nancy Magnus states that the Village needs to pass a
Supplemental Resolution in the amount of $140,536.42 (the difference between the final amount and
the original resolution). Therefore, I have prepared the attached Supplemental Resolution for your
approval.
This item is being placed on the Village Board agenda of January 8, 2002 for your consideration.

3. Trustee Caleel asked for an analysis of costs and charges for ambulance
services. Manager Veitch described the premise behind the Village's current
fees.
Mr. Philip Mayer, 177 Briarwood Loop, spoke to the process of selecting
replacement Trustees. He recognized the qualifications of the recently
nominated individuals and questioned why the nominations were not
confirmed. He felt that negative votes should be explained, because he felt that
without an explanation, it could be interpreted that the Trustee knew something
negative about the individual and that information should be relayed to the
residents.
President Bushy commented on the death of Joseph Fullan over the holidays.
She spoke of his commitment to the Village and the new Library, her personal
feelings and announced that at the dedication of the new Library the flag would
be flown at half -mast in his honor.

Motion by Trustee Caleel, seconded by Trustee Korin, to approve the minutes

Dear Ms. Gonnella:
The Supplemental Resolution for Section 99- 00031 -00 -FP adopted by the
President and Board of Trustees on January 8, 2002 was approved by this
Department on February 1, 2002. This resolution provides for resurfacing of
Adams Road, Castle Drive, Forest Gate Road, Merry Lane and Regent Drive
and appropriates an additional $140,536.42 of motor fuel tax funds for the
subject section.
With the additional funds appropriated above, a total of $640,536.42 of Motor
Fuel Tax funds have been appropriated for the subject project.
