The Contractor/Bidder shall acknowledge in writing on his bid proposal form the receipt of this Addendum.This Addendum shall be part of the Specifications and Drawings for this project and shall be part of theactual contract document to complete the work. When the Architect issues an Addendum, it is the Bidder’sresponsibility to copy and insert it into the bid documents they have obtained from the Architect or Owner.
There are six (6) items in this Addendum.
ITEM NO. 1: PROJECT MANUAL, CLARIFICATION:
The General Contractor will be responsible to apply for all required permits. This includes a permitfrom the Village of Oak Brook. However, the Village of Oak Brook will waive their normal andcustomary permit fees.
ITEM NO. 2:PROJECT MANUAL, SCHEDULE CLARIFICATION:
It is imperative that no work begin until all materials and equipment needed to complete all work areon site. When work begins on May 17, 2016, the demolition work can be completed at one time,but the new flooring work will need to be divided into two phases. Phase one will include roomnumbers 101, 104, 106, 107, 108, 109 and 110. Phase two will include room number 103.
ITEM NO. 3:PROJECT MANUAL, SECTION 09 90 00, PAINTING:
On page 09 90 00-7, under 2.04 FINISHES AND COLORS: Delete items C. and D.
ITEM NO. 4:DRAWINGS, SHEET A0.1, CLARIFICATIONS:
At the intersection of rooms 106 and 107, remove the first row (approximately 7.5 lineal feet) of theexisting ceramic tile, and replace with rubber tile. Also, provide new VCB at adjacent casework.
ITEM NO. 5:DRAWINGS, SHEET A2.1, CLARIFICATIONS:
1. At all openings into the new bunk areas (6 such), provide a double metal stud brace acrossthe top of the opening. This shall be finished with stained wood trim on the sides and thebottom, to match the wood trim shown on sheet A2.1, and the wood cap detail as shown on1/A2.1 shall continue across the top of these openings.

ITEM NO. 5: Cont.
2. Several of the elevations on this sheet show 4" vinyl base. All new walls shall receive 8" woodbase as shown on detail 3/A2.1. Also, the 4" base units below the new cabinetry shall befinished to match the cabinets, so they do not need 4" vinyl base. All existing brick walls willnot receive base, so the new carpet must be cut tight to it. Any other areas not mentionedabove shall receive new 4" vinyl base, and the few areas with existing vinyl base shall beremoved and replaced.
ITEM NO. 6:DRAWINGS, SHEET E1.1, Under GENERAL NOTES, Item 11 shall be as follows:
The F1 light fixture shall be a ‘Light Swing Arm Wall Mtd. Fixture’ as by Wayfair/Elk Lighting orequivalent.
