Oak Brook, IL 60523 October 17, 2012 Scope of Work The following is included in the installation of a FuelMaster 3500 Plus fuel management system and dispenser at the Oak Brook Public Works facility located at 3003 Jorie Blvd. Oak Brook: • Remove and dispose of existing dispenser and fuel management system. • Supply and install a Bennett 3000 series dual product dispenser with electronic registers to include: • Balanced vapor recovery equipment for gasoline. • Stainless steel panels. • Unleaded balanced vapor recovery nozzle, hose, whip hose and breakaway. • Diesel nozzle, hose, whip hose, and breakaway. Supply and install a FuelMaster 3500P1us fuel management system equipped with: • Fuel island terminal (FMU) with pedestal. • ProKee encoder (requires USB port on PC) • (100) Black ProKees. • FuelMaster Plus Microsoft Windows-based PC software. • (2) AIM nozzle tags. • (25) AIM2 vehicle kits. • Equipment to be installed reusing existing wiring. • Install PC software on owner's PC. • Provide up to 4 hours of training for owners personnel. • Start-up system and test system for proper operation. • Provide training on AIM2 installations to owner's personnel. *Does not include any local permit fees. PROJECT DESCRIPTION: Fuel Dispenser and Monitoring System Replacement 4. PREPARATION AND SUBMISSION OF BID: A. There is no pre-bid meeting. The bid must be delivered to the office of the Purchasing Clerk, Village of Oak Brook, 1200 Oak Brook Road, Oak Brook, IL 60523 on or before Tuesday, October 19, 2012, at 10:00 A.M., prevailing time, at which time it will be publicly opened and read in the S. The contractor further declares that he has carefully examined this entire Bid Package and he has inspected in detail the site of the proposed work, and that he has familiarized himself with all of the local conditions affecting the contract and the detailed requirements of this work and understands that in making the bid he waives all rights to plead a misunderstanding regarding same. I. The contractor further understands and agrees that if his bid is accepted, he is to furnish and provide all necessary machinery, tools, apparatus, and other means to do all of the work and to furnish all of the materials specked in the contract, except such materials as are to be furnished by the owner (Village), in the manner and at the time therein prescribed, and in accordance with the requirements therein set forth. The contractor declares that he understands that the quantities listed on the Bid are approximate only and that they are subject to increase or decrease; and that he will take in full payment thereof the amount and the summation of the actual quantities, as fully determined by the Village. K. The contractor further agrees that the price submitted within the bid is for the purpose of obtaining a gross sum, and for use in computing the value of extras and deductions. If there is a discrepancy between the gross sum bid and that resulting from the summations of the quantities multiplied by the unit price, the latter shall apply. L. The contractor further agrees that if the Village decides to extend or shorten the work, or otherwise alter it by extras or deductions, including elimination of one or more of the items, as provided in the specifications, he will perform the work as altered, increased or decreased, at the contract unit price. M. The contractor further agrees that the Village representative may at any time during the progress of the work covered by this Contract, order other work or materials incidental thereto and that all such work and materials as do not appear in the bid or contract as a specific item covered by a lump sum price, and which are not included under the bid price for other items in the Contract, shall be performed as extra work, and compensation shall be as set forth in the specifications. N. The contractor further agrees to execute all documents within this Bid Package, obtain a Certificate of Insurance for this work and present all of these documents within fifteen (15) days after the receipt of the Notice of Award and the Contract by him. O. The contractor further agrees that he and his surety will execute and present within fifteen (15) days after the receipt of the Notice of Award and the Contract, a Contract bond satisfactory to and in the form prescribed by the Village, in the penal amount of 100% of the Contract amount, guaranteeing the faithful performance of the work and payment for labor, material supplies, and subcontractors in accordance with the terms of the Contract. P. The contractor further agrees to begin work not later than ten (10) days after receipt of the Notice to Proceed, unless otherwise provided, and to execute the work in such a manner and with sufficient materials, equipment and labor as will insure its completion within the time limit specified within the Bid, it being understood and agreed that the completion within the time limit is an essential part of the contract. Q. By submitting a Bid, the contractor understands and agrees that, if his bid is accepted, and he fails to enter into a contract forthwith, he shall be liable to the Village for any damages the Village may thereby suffer. R. No bid will be considered unless the party offering it shall furnish evidence satisfactory to the Village that he has necessary facilities, ability and pecuniary resources to fulfill the conditions of the Contract. S. If the contractor is in doubt as to the true meaning of any part of the plans, specifications or other proposed contract documents, he may submit to the Public Works Director a written request for an interpretation thereof. The person submitting the request will be responsible for its prompt delivery. Any interpretation of the documents will be made only by addendum duly issued by the Village. The Village will not be responsible for any other explanation or interpretation of the Bid Package. 5. SUBSTITUTIONS: A. Certain materials and equipment are specified by a manufacturer or trade name to establish standards of quality and performance and not for the purpose of limiting competition. Bidders are invited to submit bids not only on named items but also on items which they propose for substitution of named items. Products of other manufacturers may be substituted, if, in the opinion of the Village, Page 2 they are equal to those specified in quality, performance, design and suitability for intended use. Where two or more items are specified, the selection among those specified is the bidder's option, or he may submit his bid on all such items. B. Bids shall be based on materials included in the specifications. Substitutions for the purpose of evaluating bids will be considered only if proposed substitutions are set forth in the sealed bid and will only be accepted prior to the award of the contract. The offer of substitutions shall be an integral part of the bid, appearing immediately after all requested bids and before the signature of the bidder. C. Substitutions of materials other than those specified will not be considered in the base bid price. However, other substitutions may be listed in the specified place in the Bid Form, with the indication of the change in the base bid price for the total cost. D. In addition to the requirements heretofore mentioned, in order for substitutions to qualify for consideration, the following shall accompany each bid: 1. Each proposed substitution shall be itemized showing manufacturer name, catalog number, quantity, unit cost and total cost. The bidder shall prepare the necessary forms to list his substitutions in the manner outlined. 2. Each bid offering substitutions shall be accompanied by descriptive literature, catalog data, complete technical specifications and reports of all pertinent tests concerning the bidder's proposed substitutions. 6. BASIS OF AWARD: The Village of Oak Brook reserves the right to reject any or all bids and to waive any informality or technical error and to accept any bid deemed most favorable to the interests of the Village of Oak Brook. In addition to price, the Village will consider: A. Ability, capacity and skill to fulfill the contract as specified. B. Ability to supply the commodities, provide the services or complete the construction promptly, or within the time specified, without delay or interference. C. Character, integrity, reputation, judgment, experience and efficiency. D. Quality of performance on previous contracts. E. Previous and existing compliance with laws and ordinances relating to the contract. F. Sufficiency of financial resources. G. Quality, availability and adaptability of the commodities, services or construction, in relation to the Village's requirements. H. Ability to provide future maintenance and service under the contract. I. Number and scope of conditions attached to the bid/proposal. J. Record of payments for taxes, licenses or other monies due the Village Page 3 CONDITIONS: The Village is exempt from Federal excise tax (#36-600-9534) and the Illinois Retailer's Occupation Tax (#E99974381-06). This bid cannot include any amounts of money for these taxes. 8. WARRANTY: Included in Section D, Specifications. 9. PAYMENT: The Village of Oak Brook authorizes the payment of invoices on the second and fourth Tuesday of the month. For consideration on one of these dates, payment request must be received no later than fourteen (14) days prior to the second or fourth Tuesday of the month. 10. INDEMNIFICATION: The Contractor shall protect, indemnify, save, defend and hold forever harmless the Village and/or its officers, officials, employees, volunteers and agents from and against all liabilities, obligations, claims, damages, penalties, causes of action, costs and expenses, including without limitation court costs, insurance deductibles and attorney's fees and expenses, which the Village and/or its officers, officials, employees, volunteers and agents may incur, suffer or sustain, or for which the Village and/or its officers, employees and agents may become obligated by reason for any accident, injury to or death of persons or loss of or damage to property, or civil and/or constitutional infringement of rights (specifically including violations of the Federal Civil Right Statutes), arising indirectly or directly in connection with or under, or as a result of, this or any Agreement by virtue of any act or omission of any of the Contractor's officers, employees, subcontractors, and/or agents, provided that the Contractor shall not be liable for claims, obligations, damages, penalties, causes of action, costs and expenses arising solely by any act or omission of the Village's officers, officials, employees, volunteers and/or agents. The contractor shall hold the Village harmless for any and all claims for labor, material, apparatus, equipment, fixtures or machinery furnished to the contractor for the purpose of performing the work under the contract; and the payment of all direct and indirect damages to any person, firm, company or corporation suffered or sustained on account of the performance of such work during the time the contract is in force. 11. INSURANCE: Certificates of Insurance shall be presented to the Village within fifteen (15) days after the receipt by the contractor of the Notice of Award and the unexecuted contract, it being understood and agreed that the Village will not approve and execute the contract until acceptable insurance certificates are received and approved by the Village. Each contractor performing any work pursuant to a contract with the Village of Oak Brook and each permittee working under a permit as required pursuant to the provisions of Title 1 of Chapter 8 of the Code of Ordinances of the Village of Oak Brook (hereinafter referred to as "Insured") shall be required to carry such insurance as Specified herein. Such contractor and permittee shall procure and maintain for the duration of the contract or permit insurance against claims for injuries to persons or damages to property which may arise from or in connection with the performance of the work under the contract or permit, either by the contractor, permittee, or their agents, representatives, employees or subcontractors. A contractor or permittee shall maintain insurance with limits no less than: Page 4 A. General Liability - $2,000,000 combined single limit per occurrence for bodily injury, personal injury and property damage, provided that when the estimated cost of the work in question does not exceed $5,000, the required limit shall be $500,000; B. Automobile Liability (if applicable) - $1,000,000 combined single limit per accident for bodily injury and property damage; C. Worker's Compensation and Employer's Liability - Worker's Compensation limits as required by the Labor Code of the State of Illinois and Employer's Liability limits of $1,000,000 per accident. Any deductibles or self-insured retention must be declared to and approved by the Village. At the option of the Village, either the insurer shall reduce or eliminate such deductible or self-insured retention as respects the Village, its officers, officials, employees and volunteers; or the Insured shall procure a bond guaranteeing payment of losses and related investigations, claim administration and defense expenses to the extent of such deductible or self-insured retention. The policies shall contain, or be endorsed to contain, the following provisions: 0. General Liability and Automobile Liability Coverage - (1) The Village, its officers, officials, employees and volunteers are to be covered as additional insureds as respects: liability arising out of activities performed by or on behalf of the Insured; premises owned, occupied or used by the Insured. The coverage shall contain no special limitations on the scope of protection afforded to the Village, its officers, officials, employees, volunteers, or agents. (2) The Insured's insurance coverage shall be primary insurance as respects the Village, its officers, officials, employees, volunteers and agents. Any insurance or self-insurance maintained by the Village, its officers, officials, employees, volunteers or agents shall be in excess of the Insured's insurance and shall not contribute with it. (3) Any failure to comply with reporting provisions of the policies shall not affect coverage provided to the Village, its officers, officials, employees, volunteers or agents. (4) The Insured's insurance shall apply separately to each covered party against whom claim is made or suit is brought except with respect to the limits of the insurers liability. E. Worker's Compensation and Employer's Liability Coverage The policy shall waive all rights of subrogation against the Village, its officers, officials, employees, volunteers and agents for losses arising from work performed by the insured for the Village. Each insurance policy shall be endorsed to state that coverage shall not be suspended, voided, canceled by either party, reduced in coverage or in limits except after thirty (30) days prior written notice by certified mail has been given to the Village. Each insurance policy shall name the Village, its officers, officials and employees, volunteers and agents as additional Insureds. Insurance is to be placed with insurers with a Best's rating of no less than A: VI I. Each Insured shall furnish the Village with certificates of insurance and with original endorsements effecting coverage required by this provision. The certificate and endorsements for each insurance policy are to be signed by a person authorized by that insurer to bind coverage on its behalf. The certificates and endorsements are to be on forms approved by the Village and shall be subject to approval by the Village Page 5 Attorney before work commences. The Village reserves the right to require complete, certified copies of all required insurance policies, at any time. Each insured shall include all subcontractors as insureds under its policies or shall furnish separate certificates and endorsements for each subcontractor. All coverages for subcontractors shall be subject to all of the requirements stated herein. 12. SAFETY: The contractor and any subcontractors shall comply with all the provisions of the Federal Occupational Safety and Health Act of 1970 (84 Stat. 1590), as amended. 13. EQUAL OPPORTUNITY: The Contractor will not discriminate against any employee or applicant for employment because of race, color, religion, sex, ancestry, national origin, place of birth, age or handicap unrelated to bona fide occupational qualifications. 14. PREVAILING RATE OF WAGES: All wages paid by the Contractor and each subcontractor shall be in compliance with The Prevailing Wage Act (820 ILCS 130), as amended, except where a prevailing wage violates a federal law, order, or ruling, the rate conforming to the federal law, order, or ruling shall govern. The Contractor shall be responsible to notify each subcontractor of the wage rates set forth in this contract and any revisions thereto. If the Department of Labor revises the wage rates, the revised rate as provided by the public body shall apply to this contract and the Contractor will not be allowed additional compensation on account of said revisions. Contractor will comply with the Illinois prevailing wage law, as amended from time to time. Not less than the prevailing rate of wages as found by Owner or the Illinois Department of Labor shall be paid to all laborers, workers and mechanics performing work under the Contract. If the Department of Labor revises the prevailing rate of wages to be paid laborers, workers or mechanics under the Contract, Owner will notify Contractor and each Subcontractor of the change in the prevailing rate of wages; provided, however, regardless of whether Owner gives such notice, the revised prevailing rate of wages shall apply to the Contract and Contractor shall have the sole responsibility and duty to pay, and ensure that all Subcontractors pay, the revised prevailing rate of wages to each person to whom a revised rate is applicable. Revision of the prevailing wages shall not result in an increase in the Contract sum or other cost to Owner. Contractor shall indemnify, defend and hold Owner harmless from any loss, including but not limited to Owner's attorneys fees, resulting from Contractor's failure to comply with this prevailing wage clause. All bonds applicable to the Contract shall include a provision as will guarantee the faithful performance of the obligation to pay the prevailing rate of wages. The Contractor and each subcontractor shall make and keep, for a period of not less than 3 years, records of all laborers, mechanics, and other workers employed by them on the project; the records shall include each worker's name, address, telephone number when available, social security number, classification or classifications, the hourly wages paid in each period, the number of hours worked each day, and the starting and ending times of work each day. The Contractor and each subcontractor shall submit monthly, in person, by mail, or electronically a certified payroll to the public body in charge of the project. The certified payroll shall consist of a complete copy of the records. The certified payroll shall be accompanied by a statement signed by the contractor or subcontractor which states that: (i) such records are true and accurate; (ii) the hourly rate paid to each worker is not less than the general prevailing rate of hourly wages required; and Page 6 (iii) the contractor or subcontractor is aware that filing a certified payroll that he or she knows to be false is a Class B misdemeanor. Upon 2 business days' notice, the contractor and each subcontractor shall make available for inspection the records to the public body in charge of the project, its officers and agents, and to the Director of Labor and his deputies and agents at all reasonable hours at a location within this State. The Contractor and each subcontractor shall permit his/her employees to be interviewed on the job, during working hours, by compliance investigators of the Department or the Department of Labor. 15. EMPLOYMENT OF ILLINOIS WORKERS DURING PERIODS OF EXCESSIVE UNEMPLOYMENT: The Contractor shall comply with all Illinois statutes pertaining to the selection of labor Whenever there is a period of excessive unemployment in Illinois, which is defined herein as any month immediately following 2 consecutive calendar months during which the level of unemployment in the State of Illinois has exceeded 5 percent as measured by the United States Bureau of Labor Statistics in its monthly publication of employment and unemployment figures, the Contractor shall employ only Illinois laborers. "Illinois laborer" means any person who has resided in Illinois for at least 30 days and intends to become or remain an Illinois resident. Other laborers may be used when Illinois laborers as defined herein are not available, or are incapable of performing the particular type of work involved, if so certified by the Contractor and approved by the Municipality. The Contractor may place no more than 3 of his regularly employed non-resident executive and technical experts, who do not qualify as Illinois laborers, to do work encompassed by this Contract during a period of excessive unemployment. This provision applies to all labor, whether skilled, semi -skilled or unskilled, whether manual or non - manual. 16. COPIES OF DOCUMENTS The number of copies of Contract and Bond required to be executed is as follows: a) Two (2) original counterparts of the Contract documents will be required to be executed. 17. EXECUTION OF DOCUMENTS The Contractor, in signing his Bid on the whole or on any portion of the work, shall conform to the following requirements: A Bid signed by an individual other than the individual represented in the Bid documents shall have attached thereto a power of attorney evidencing authority to sign the Bid in the name of the person for whom it is signed. A bid signed for a partnership shall be signed by all of the partners or by an attorney-in-fact. If signed by an attorney-in-fact, there shall be attached to the Bid a power of attorney evidencing authority to sign the bid, executed by the partners. A bid signed for a corporation, shall have the correct corporate name thereof and the signature of the President or other authorized officer of the corporation manually written below the corporate name. If such Bid is manually signed by an official other than the President of the Corporation, a certified copy of a resolution of the board of directors evidencing the authority of such official to sign the Bid should be Page 7 18. 19. 20. 21 attached to it. Such Bid shall also bear the attesting signature of the Secretary of the corporation and the impression of the corporate seal. The Contract shall be deemed as have been awarded when formal notice of award shall have been duly served upon the intended awardee. NON -DISCRIMINATING: The Vendor, its employees and subcontractors, agrees not to commit unlawful discrimination and agrees to comply with applicable provisions of the Illinois Human Rights Act, the U.S. Civil Rights Act and Section 504 of the Federal Rehabilitation Act, and rules applicable to each. INDEPENDENT CONTRACTOR: There is no employee/employer relationship between the CONTRACTOR and the VILLAGE. CONTRACTOR is an independent contractor and not the VILLAGE'S employee for all purposes, including, but not limited to, the application of the Fair Labors Standards Act minimum wage and overtime payments, Federal Insurance Contribution Act, the Social Security Act, the Federal Unemployment Tax Act, the Worker's Compensation Act (820 ILCS 305/1, et seq.). The VILLAGE will not (i) provide any form of insurance coverage, including but not limited to health, worker's compensation, professional liability insurance, or other employee benefits, or (ii) deduct any taxes or related items from the monies paid to CONTRACTOR. The performance of the services described herein shall not be construed as creating any joint employment relationship between the CONTRACTOR and the VILLAGE, and the VILLAGE is not and will not be liable for any obligations incurred by the CONTRACTOR, including but not limited to unpaid minimum wages and/or overtime premiums, nor does there exist an agency relationship or partnership between the VILLAGE and the CONTRACTOR. ASSIGNMENT Neither the VILLAGE nor the CONTRACTOR shall assign or transfer any rights or obligations under this Agreement without the prior written consent of the other party, which consent shall not be unreasonably withheld. GOVERNING LAW This Agreement shall be governed by the laws of the State of Illinois as to interpretation, performance and enforcement. The forum for resolving any disputes concerning the parties' respective performance or failure to perform under this Agreement shall be the Circuit Court for the Eighteenth Judicial Circuit, DuPage County, Illinois. Page 8 Section D (REVISED) Specifications Public Works Building Fuel Dispenser and Monitoring System Replacement PART1-GENERAL 1.1 SUMMARY OF THE WORK 1.1.1 CONTRACT DOCUMENTS AND RELATED REQUIREMENTS Drawings, general provisions of the contract, including general and supplementary conditions and other Division 01 specifications, shall apply to the work of this section. The contract documents show the work to be done under the contract and related requirements and conditions impacting the project. Related requirements and conditions include applicable codes and regulations, notices and permits, existing site conditions and restrictions on use of the site, requirements for partial owner occupancy during the work, coordination with other work and the phasing of the work. In the event the Contractor discovers a conflict in the contract documents and/or requirements or codes, the conflict must be brought to the immediate attention of the Owner for resolution. Whenever there is a conflict or overlap in the requirements, the most stringent shall apply. 1.1.2 EXTENT OF WORK A. In general the work shall include the removal and replacement of the existing fuel dispenser and wiring located at the Village's Public Works Building along with the replacement of the fuel management system. 1.1.3 RELATED WORK A. Section not used. 1.1.4 TASKS The work tasks are summarized briefly as follows A. Remove and provide proper disposal for the existing fuel dispenser. The base pad shall not be removed. Furnish and install a new twin product dispenser at the same location. Provide all necessary permits required for the replacement. B. Remove and provide proper disposal for the existing fuel management system. Furnish and install a new fuel management system with a capacity for a minimum of 100 users. The work shall include a management system using transponders in the vehicles, necessary cards, encoders, keys, software and training for a completely functioning fuel management system by the Village's personnel. The system shall have a minimum of 25 and a maximum of 75 transponders. Twenty-five (25) vehicles will in receive transponders, with additional transponders to be added in future years. The remaining vehicles shall operate the fuel management system with cards or keys. C. As an alternate, a new fuel management system with a capacity for a minimum of 100 users using cards or keys may be furnished and installed. The work shall include a management system using all necessary equipment, software, and training shall be provided for a completely functioning fuel management system. Page 1 1.1.5 CONTRACTOR USE OF PREMISES A. The Contractor and Contractor's personnel shall cooperate fully with the Owner's representative/consultant to facilitate efficient use of buildings and areas within building. The Contractor shall perform the work in accordance with the Owner's specifications, drawings, phasing plan and in compliance with any/all applicable Federal, State and Local regulations and requirements. B. The Contractor shall use the existing facilities in the building strictly within the limits indicated in contract documents. 1.2 APPLICABLE CODES AND REGULATIONS 1.2.1 GENERAL APPLICABILITY OF CODES, REGULATIONS, AND STANDARDS A. All work under this contract shall be done in strict accordance with all applicable Federal, State, and local regulations. All applicable codes, regulations and standards are adopted into this specification and will have the same force and effect as this specification. B. The most recent edition of any relevant regulation, standard, document or code shall be in effect. Where conflict among the requirements or with these specifications exists, the most stringent requirement(s) shall be utilized. 1.2.2 PERMITS/LICENSES A. The contractor shall apply for and have all required permits and licenses to perform the work as required by Federal, State, and Local regulations. 1.2.3 POSTING AND FILING OF REGULATIONS A. Maintain two (2) copies of applicable federal, state, and local regulations. Post one copy of each at the regulated area where workers will have daily access to the regulations and keep another copy in the Contractor's office. 1.3 OWNER'S RESPONSIBILITIES Prior to commencement of work: A. Notify occupants adjacent to the work areas of the project dates and requirements for relocation, if needed. Arrangements must be made prior to starting work for relocation of equipment. 1.4 PRE -CONSTRUCTION MEETING Prior to commencing the work, the Contractor shall meet with the Owner's representative to present and review, as appropriate, the project schedule, safety requirements submittals and access to the work area. 1.5 PROJECT COORDINATION The following are the minimum administrative and supervisory personnel necessary for coordination of the work. PART 2 - PRODUCTS, MATERIALS AND EQUIPMENT 2.1 MATERIALS AND EQUIPMENT 2.1.1 GENERAL REQUIREMENTS Prior to the start of work, the contractor shall provide and maintain a sufficient quantity of materials and equipment to assure continuous and efficient work throughout the duration of the project. Work shall not Page 2 start unless the following items have been delivered to the site and verification has been submitted to the Owner's representative. A. All materials shall be delivered in their original package, container or bundle bearing the name of the manufacturer and the brand name (where applicable). B. Store all materials subject to damage off the ground, away from wet or damp surfaces and under cover sufficient enough to prevent damage or contamination. Flammable and combustible materials cannot be stored inside buildings. C. The Contractor shall not block or hinder use of buildings by staff, and visitors to the Owner's building by placing materials/equipment in any unauthorized location. D. The Owner's representative shall inspect for damaged, deteriorating or previously used materials. Such materials shall not be used and shall be removed from the worksite and disposed of properly. E. Adequate and appropriate PPE for the project and number of personnel/shifts shall be provided. All personal protective equipment issued must be based on a written hazard assessment in accordance with OHSA requirements. 2.2 SUBMITTALS 2.2.1 PRE -CONSTRUCTION MEETING SUBMITTALS Submit to the Owner a minimum of 14 days prior to the pre -start meeting the following for review and approval. Meeting this requirement is a prerequisite for the pre -construction meeting for this project: A. Submit a detailed work schedule for the entire project reflecting contract documents and the phasing/schedule requirements from the CPM chart. B. Submit a staff organization chart showing all personnel who will be working on the project and their capacity/function. 2.3. FUEL DISPENSER The fuel dispenser shall be a GasBoy 9800 Series Twin Product Dispenser or similar. The fuel dispenser shall include the following features: 1. Twin produce side load with electronic register 2. Vapor recovery equipment 3. Stainless steel panels 4. The Unleaded Vapor Vac Nozzle shall include Nozzle, Hose, Whip Hose and Breakaway 5. The Diesel Nozzle shall include Nozzle, Hose, Whip Hose and Breakaway 2.4 FUEL MANAGEMENT SYSTEM A. FuelMaster 3500 Plus AIM 2 Fuel Management System or Similar Requirements: The specifications define the performance of a fully autonomous fuel management system. The system operator will use read/write keys (Prokees). The fuel management system will control, authorize, and record the dispensing of petroleum products as a stand-alone system, capable of unattended operation for 7 days a week, 24 hours a day. Dispensing of petroleum products is limited to vehicles and equipment with authorized read/write keys or other authorized device. Dial up of the fuel island controller will by a telephone line or alternatively by a network using TCP/IP, or by wireless method, with software loaded on a remotely located PC. (PC will be provided by Village.) Page 3 1. Electronic Read/Write Key — Prokee(s) 2. Preventative Maintenance Alert & MPG — Based on odometer/hour readings, the system calculates vehicle efficiency and fuel consumption and alerts driver and supervisor to maintenance requirements. 3. Inventory control/Tank Monitor Interface — Permits automatic reconciliation of tank level with FuelMaster's declining balance. 4. Expandability- May be expanded to control at almost limitless number of fueling sites sharing a common list of users and vehicles. 5. Warranty- At least one (1) year from date of installation. 6. Modular Electronics- Modular design and construction to ensure reliability and ease of maintenance. Site operators can quickly and easily change out modular components, if necessary, with the assistance of FuelMaster technicians. 7. Central Controller- Comprehensive transaction reports and invoices are generated by the Central Controller operating on a PC that uses either Windows XP or 7. Transaction data must be easily exported to a Fleet Management program. B. Performance: A fuel island controller, located on the fuel island, is used to turn fuel dispensers on and off, monitor fuel dispensing, receive and send RF data from recognized authorized read/write keys PROKEES and interface with dispensers and tank level monitoring systems. The fuel island control should have flash memory in order to be updated via telephone, USB, Ethernet or wireless solution. It should store between 2,000 and 4,000 fueling transactions with data to identify the vehicle, user, type of fuel, amount of fuel, date/time stamp and additional information. 2. Software should be installed on an IBM compatible PC, permitting: Manipulation of transaction data for printing reports on vehicles, users, products, etc.; Downloading of transactions and uploading of authorized vehicle and user lists; and Transfer and storage of data Software should be capable of transferring transaction data in a standard format (including, but not limited to: flat ASCII file, comma delimited, XML). Software should include an invoicing capability that allows the user to generator invoices for selected customers, departments, or agencies from the Central Controller printer. Invoices should list each transaction for all vehicles for a user - selected period. Data from non -fueling sites must be integrated into the fueling site data to provide odometer/hour or error code information on vehicles that have not fueled at the controlled site. Software must have the capability to be used on a network with multiple seats. This software must be compatible with Windows operating systems (Windows XP and 7). 3. System configuration. The fuel island equipment should be configured to authorize and control the dispensing of fuel based on the information supplied to the fuel island controller by the read/write keys. This data and the fuel island controller stored data are processed by the fuel island controller prior to authoring a fueling operation. Operators will use keys, cards, and/or RFIDs for access. Upon completion of a fueling operation, a fueling transaction (time, date, hose number, quantity, transaction type, vehicle ID, and odometer) is recorded by the fuel island controller for later transfer to the software. 4. System Capability. The fuel island controller must have the capability to simultaneously control up to four (4) hoses. The fuel island unit should also have the capaibility to control alternative fuels or liquid products. The fuel island controllers must have a time-out to deactivate a hose should a pulse count and/or fueling stop. This missing pulse detector should be a variable with the supervisor being able to set it as required. Page 4 C. Data Management and Reporting: Fuel island controller should be downloaded by the Central Controller operator at the operator's convenience or at a time of day programmable in the software by the Central Controller operator. The software should be capable of unattended dialing and downloading when the site is not in use. When automatic downloading, the software should dial the site and return to the site if the modem was busy on the first or second attempts. Following the downloading, transaction data is available for report generation. 2. Software should provide the following information at the Central Controller as a transaction record: • Vehicle odometer and/or chronometer • Vehicle ID number • User ID if collected • Transaction number • Number of units (gallons, quarts, cubic feet, etc.) dispensed to tenths or hundredths • Fuel site • Date & Time • Hose number • Product number • Engine idle time • PTO time • Engine error messages From this information, brought back to the Central Controller, much additional information may be determined. Using the last and current odometer, the vehicle efficiency or MPG should be determined. The above listed data permits the collection of fleet data to be used as a management tool. 3. The software should be capable of totaling monthly fuel costs by organization number, vehicle ID, agency number or by employee. The software must allow the operator to be able to compile reports for all transaction by site, organization, date, vehicle, etc. 4. The software should keep a declining balance inventory of the fuel remaining in the tanks. Tank inventory must be maintained. The software should also provide a flag when the fuel has reached a certain level and should know what should be purchased for a specific tank. 5. Exception reports for vehicles needing maintenance, with out of range odometers, or exceeding certain fuel limitations should be printed and downloaded. D. Storage: In the event of a power failure, the system should have the capability to store all data collected up to the time of the power failure for minimum period of three (3) months. The fuel island controller at the fueling site should have the ability to operate if the Central Controller is down, limited only by the fuel controller's internal storage capacity. There should be a method to access transaction information should there be data transmission problems. E. Additional Items: True Manual Override- The fuel island controller must permit manual override of the fuel island controller should any problem occur. The override must be complete, total -by-pass of the fuel island controller. Page 5 2. Semi -Manual Mode- The fuel island controller must have the capability to record fuel dispensed in emergency situations when there is a need for vehicles without keys to be refueled and yet fuel accountability is still desired. When in this mode, fuel can be dispensed by any pup as if the fuel island controller was not functioning, but the fuel island controller will record all transactions as semi -manual transactions. 3. Services- The fuel island controller equipment should be designed to operate from 120 VAC, 60 HZ single phase power. 4. Extended Maintenance Agreements- The vendor providing the equipment may provide an extended maintenance agreement for the equipment and software. F. Safety: Fuel management system equipment must be provided with all the necessary safety devices and guards to protect the operator. All primary operator control buttons, switches, etc., must be grouped and mounted in a location affording the operator convenient access to the controls. Essential safety operating instructions must be identify safety and health hazards associated with the equipment, procedures, and practices necessary for safe operations. Placards must be provided to warn operator or maintenance personnel of hazardous conditions, and warnings. G. Training: Qualified instructors must provide on-site training of personnel in the functions of operation, maintenance, and repair as they apply to each specific item of equipment. Training on the software will be conducted at a specific computer site location. H. Equipment Requirements: • 1 ea- FuelMaster Fuel Island Controller with PROKEE acceptor or similar • 1 ea- Fuel Master 3500 AIM 2 Windows XP or 7 compatible software or similar • 1 ea- PROKEE Encoder or similar 100ea- PROKEES or similar e 25ea- AIM 2 Transponders or similar Equipment furnished must be designed to meet the following specifications: National Electric Code, NFPA #70-2003 Uniform Fire Code Underwriters Laboratories UL1238 and 913 FCC Part 15, Class A National Weights and Measures Certification I. Contract Terms on Maintenance and Spare Parts: 1. Spare parts. Vendor may agree to sell spare parts for the operating life of the equipment, estimated to be at least 10 years. 2. If provided, the bidder will provide contract pricing for parts and labor that will be established for approximately 10 years. This contract will cover any work or repairs needed that do not fall under warranty work on the equipment proposed. 3. Maintenance Agreement. The vendor may agree to provide system maintenance on a yearly renewable contract for the life of the system. Page 6 A. If provided, the bidder should provide a support plan, and a preventative maintenance, (including, but not limited to version updates), outline with cost on proposed system. J. Miscellaneous Requirements: 1. Provide a sample software license agreement and contract language, if available. 2. Provide training/user manuals with response. 3. Provide any freight or delivery charges, if applicable. 4. Submit a schedule for completion of work. 2.5 ALTERNATE FUEL MANAGEMENT SYSTEM: As an alternate, the FuelMaster 2500 Plus fuel management system or similar may be provided. The features of the system shall be similar to those as listed in Section 2.4 with the deletion of the vehicle transponders and the software needed for the transponders. Under this alternative, fuel management system shall include the following features: Card reader with pedestal Network Card a PROKEE Encoder or similar • 100 Prokees or similar • FuelMaster Plus Windows Software or similar PART 3 - EXECUTION 3.1 FUEL DISPENSER 3.1.1 REMOVAL OF EXISTING DISPENSER The Contractor shall remove and provide proper disposal of the existing fuel dispenser. The existing concrete pad shall not be removes and no piping below the breakaway of the existing dispenser shall be removed. 3.1.2 NEW FUEL DISPENSER The Contractor shall install the new fuel dispenser on the existing fuel dispenser pad. All necessary piping and electrical connections shall be performed by the Contractor for proper installation and operation of the new fuel dispenser. 3.2 FUEL MANAGEMENT SYSTEM 3.2.1 REMOVAL OF EXISTING FUEL MANAGEMENT SYSTEM The Contractor shall remove and provide proper disposal for the existing fuel management system including the terminal, pedestal, and card connecting wiring. 3.2.2 NEW FUEL MANAGEMENT SYSTEM The Contractor shall install the new fuel management system terminal on the existing fuel dispenser island. Use existing power and communication wiring for the new card reader and the designated PC for operation of the fuel system, unless alternatives are provided. Furnish and install new card reader/encoder and all software necessary for the encoder. Page 7 The New Fuel Management System shall be capable of allowing the Village to add, delete or modify user the cards for the system. This shall be provided by using either a new card reader/encoder or a software program that allows using the Village's existing readertencoder. If provided, the system shall be capable of reprogramming the transponder for use with a different vehicle, if selected. 3.2.3 TESTING The Contractor shall test the completed system for performance of the management system and compatibility with the Village's network. 3.2.4 TRAINING The Contractor shall provide sufficient training for the Village personnel to satisfactorily operate the new fuel management system, including training Village staff on the installation of 10 transponders. If an alternate fuel system is provided that does not use transponders, deduct the training on 10 transponders. 3.2.5 WORK HOURS All work shall be done during administrative hours (7:00 A.M. to 3:00 P.M.) Monday - Friday excluding Federal Holidays. Any change in the work schedule must be approved by the Owner. 3.2.6 WARRANTY All material and workmanship shall be warranted against defects in material and labor for one (1) year after acceptance of the project by the Owner. 3.2.7 MAINTENANCE AND SUPPORT If provided, a support plan with preventative maintenance (including, but not limited to version updates), for up to nine (9) years additional years beyond the initial year, should be detailed. Page 8 16009 BASIC ELECTRICAL MATERIALS AND METHODS PART 1 -GENERAL 1.01 CODES AND STANDARDS A. All electrical work to be performed and all materials to be furnished shall be in accordance with the rules and regulations of the National Fire Protection Association, National Electric Code, the State and Local Codes, the Contract Specifications, and to the satisfaction of the Engineer. 1.02 CONNECTIONS AND IDENTIFICATION OF CIRCUITS A. Included in this Contract shall be all necessary approved terminal and terminating devices required for a complete and working installation, satisfactory to the Engineer. Where conductors are to be terminated in existing equipment, the Electrical Contractor shall obtain all required termination information and complete all terminations. All wires and cables shall be banded with an identifying number at each end termination and at each splice or pull point in junction boxes and pull boxes. The identifying number of each wire shall be determined at the point of circuit origination, and shall continue unchanged to the point of circuit termination. 1.03 MATERIALS A. No materials of any kind shall be used that have not been approved by the Underwriter's Laboratories, Inc., where UL provides such service, and each piece of equipment shall have marked thereon, where it can readily be observed, the name or trademark of the manufacturer. PART 2 -PRODUCTS 2.01 ALUMINUM RIGID CONDUIT A. Aluminum conduit shall be rigid, manufactured of 6063 alloy in temper designation T-1. Fittings shall be of the same alloy. Conduit shall conform to Federal Specifications WW -C -0054 -DC and ANSI C- 80.5. B. Conduit shall be lined with a silicone compound to reduce friction and drag. 2.02 OUTLET BOXES AND FITTINGS A. Outlet boxes and fittings shall be of proper dimensions for each application, complete with watertight gaskets and covers secured with stainless steel screws. B. Conduit fittings, such as elbows, tees, couplings, caps, bushings, nipples, and locknuts shall be threaded to provide a watertight connection. C. Joints in conduit and between conduit and fittings shall be watertight and ends shall be reamed to prevent damage to conductor insulation. D. A non -corrosive, conductive thread lubricant, such as "STL" thread lubricant as manufactured by Crouse -Hinds shall be thoroughly applied on all threaded joints. E. Cast metal outlet boxes shall be Crouse -Hinds Company, Square D Company, or equal, and shall be coated inside and outside with corrosion -resisting epoxy or equal finish. F. All junction boxes, fittings or other terminals without tapped conduit entrances shall be provided with double nuts and standard bushings. G. In hazardous locations all outlet boxes and fittings shall be NEMA Type 7. Seal fittings shall be supplied in accordance with the NEC. 2.03 TERMINAL, JUNCTION AND PULL BOXES A. Terminal, junction, and pull boxes shall be installed as required. B. In general, boxes shall be made of the same material used in the conduit run. Boxes made of cast iron or cast aluminum shall be used, unless otherwise specified. Where the weight of cast boxes exceed 50 pounds, boxes may be made of 1/8 inch sheet aluminum or of (minimum) No. 12 gauge stainless steel, Page 9 with sides flanged around the cover opening or with approved supporting frame for cover. Mounting lugs and threaded entry bosses shall be provided as required. Seams in sheet metal boxes shall be continuously welded and ground smooth. C. Pull and junction boxes shall have covers held in place by stainless steel screws. Terminal boxes shall have minimum 12 gauge steel panels for mounting terminal blocks. Hinged covers shall fit tightly against a gasket, secured by lug bolts and wing nuts. All boxes shall be provided with rabbeted gaskets or flange gaskets, securely held in place. D. Cast metal terminal, junction, and pull boxes shall be Crouse -Hinds Company, Square D Company, or equal, and coated inside and outside with corrosion -resisting epoxy or equal finish. E. In hazardous locations, all terminal junction boxes and pull boxes and fittings shall be NEMA Type 7. 2.04 FLEXIBLE CONDUIT A. Flexible conduit shall be liquidtight electrical conduit having a core of flexible, steel tubing covered with a liquidtight polyvinyl jacket. Tubing shall be fitted with grounding bushings and standard rigid conduit fittings for connection and attachment. Conduit shall be Electric -Flex Company, Type HTA, Anaconda, or equal. B. In hazardous locations flexible metallic conduits and fittings shall be NEMA Type 7. 2.06 GROUNDING A. The hereinbefore described and specified electrical equipment and neutral of wiring system shall be permanently and securely grounded in accordance with the latest requirement of National Electrical Code. B. Insulated copper conductors for equipment grounding shall be routed with all power conductors and shall be sized in accordance with the National Electric Code Table 250-95. These conductors are required for all equipment connected under this Contract; including fixtures, receptacles, and electrical equipment furnished by others, even if not shown on the Contract Drawings. C. Test the ground resistance of the grounding system and provide copies of all grounding system tests for review by the Engineer and for inclusion in the Operation and Maintenance Manuals, 2.06 WIRES AND CABLE (600 VOLTS) A. A complete system of insulated copper conductors shall be installed in the conduit system, except where otherwise designated. Unless otherwise specified, conductors shall be Type XHHW 600V cross- linked polyethylene type insulation. All conductors shall conform to the requirements of the National Electric Code of IPCEA. B. Except where otherwise noted, all power conductors shall be single conductor #12 AWG minimum, and all control conductors shall be stranded single conductor #14 AWG minimum. Wire and cable shall be as manufactured by Okonite Company, Pirelli Cable Corp., or equal. C. Conductor Testing and Warranty 1- All conductors shall be tested for continuity. In addition, all power conductors shall be meggered to indicate compliance with the manufacturer's guaranteed values. A summary of test results shall be submitted for record purposes. 2. All conductors rated 600 V and below shall be furnished with a manufacturer's one year warranty which shall be submitted at the time of cable approval and commence on the date of testing. Page 10 PART 3 - EXECUTION 3.01 INSTALLATION OF CONDUITS, FITTINGS, AND BOXES A. All conduit fitting and boxes such as elbows, tees, couplings, caps, bushings, nipples, junction boxes, and lock nuts shall be threaded to provide a watertight connection. No box shall be drilled and tapped for more conduits than actually enter it and all box covers shall be accessible after installation. B. All conduits shall be installed as required. The conduit system shall be installed complete with all accessories, fittings, and boxes, in an approved workmanlike manner to provide proper raceways for electrical conductors. C. All exposed conduits shall be run parallel to or at right angles to walls or beams, and plumb on the walls. D. As far as practicable, conduit shall be pitched slightly to drain to the outlet boxes, or otherwise installed to avoid trapping of condensate. Where necessary to secure drainage, a Crouse -Hinds Company Type ECD, Square D Company, or equal breather -drain fitting shall be installed in the boxes or trapped conduit at low points. Conduit shall not be run through columns or beams unless so specifically detailed on the Contract Drawings. E. Conduit system bends and offsets may be made in field using approved bending tools, but no deformed, split or crushed conduit will be permitted in the work. All bends in conduit over 1 inch in diameter shall be made with a pipe bending machine. No more than three quarter bends shall be made between any two pull boxes without permission of the Engineer. F. Conduits shall be installed throughout structures in a completed system and must be so run that electrical conductors can be withdrawn and replaced at any time. G. Where existing conduits are to be reused, they shall be cleaned using approved methods removing all obstructions or imperfections liable to injure the new conductor insulation. H. Conduits to be built into structure shall be properly protected and suitably supported to prevent strains at joints or injury by building operations, and shall be thoroughly protected at all times from the entrance of water or other foreign matter by being well plugged when work is interrupted. If left dead ended, they shall be furnished with iron caps or pipe plugs. I. The interior of all conduits, conduit fittings, pull and junction boxes shall be carefully and thoroughly cleaned before and after erection. J. Special care shall be taken to prevent conduits from becoming choked with cement or other debris. K. No conduit smaller than 3/4 inch shall be used. 3.02 CONDUIT CONNECTIONS TO EQUIPMENT A. The conduit system shall terminate at the terminal box or at the conduit connection point of electric motor and devices. Terminations of conduits at such locations shall permit direct wire connections to the motor and devices. B. Conduit connections shall be made with rigid conduit if the equipment is fixed and not subject to adjustment, mechanical movement, or vibration. Rigid conduit connections shall have union fittings, to permit removal of equipment without cutting or breaking the conduit. C. Conduit connections shall be made with approved flexible metallic conduit if the equipment is subject to adjustment, mechanical movement, or vibration. Flexible conduit connections shall be watertight. Page 11 3.03 INSTALLATION OF CONDUCTORS A. All wires and cables pulled into conduits shall be carefully handled to avoid twists or kinks in the conductors, or damage to the sheaths and insulation. Wire and cable manufacturer's recommendations for allowable minimum bending radius and maximum pulling tension shall not be exceeded. B. All trapped conduit lines shall be swabbed to remove any debris or accumulated moisture before cables or wires are pulled in. C. No splices will be permitted between terminals, except at approved junction or terminal box points, as required by code for pull lengths. Cable and wire runs shall be looped through pull boxes without cutting and splicing, where possible. D. The neutral (current carrying ground) conductors be colored WHITE and that earth grounding conductors be GREEN. The hot lines shall be black, blue or red depending on voltage system. E. The color coding may be by the continuous external color of the insulation as applied at the factory during manufacture, or, by color coded sleeves at the discretion of the Engineer; but in no case will field applied colored tape be acceptable nor will colored sleeves be acceptable for grounding conductors or neutral conductors. 3.04 ACCEPTANCE TEST A. This test shall consist of a complete coordination and functional check under simulate operating conditions, followed by operational test under actual operating conditions. All tests that are a part of this acceptance test shall be conducted in the presence representative(s) of the Oak Brook Public Works Personnel. All costs of equipment, tools, instrument, labor, etc., to make these tests to the satisfaction of the Engineer, together with all necessary supervision by the Contractor and manufacturers' service engineers, shall be borne by the Contractor. B. Any defect in material, equipment, or workmanship furnished under this Contract, together with all inadequacies which may show up during the above tests, shall be promptly made good at the expense of the Contractor, and a new acceptance test shall be scheduled, subject to the same provisions as above described. Page 12 16010 BASIC ELECTRICAL REQUIREMENTS PART 1 -GENERAL 1.01 SUMMARY OF WORK A. As a minimum the Electrical work shall include but not be limited to the following: 1. Disconnect and remove the existing electrical equipment, conduit and wiring as required including, motor controller, starters, etc. 2. Furnish, install, connect and test motor starters, local control station, wiring, conduit extension, junction boxes and all appurtenances, for a mixer, and as necessary to furnish a complete and operating mixer, as defined in the Contract Documents. 3. Perform acceptance test and furnish documentation as hereinafter specified. 1.02 SUBMITTALS A. Submit catalog cuts and shop drawings for the electrical materials and equipment. Shop drawings shall include: 1. Schematic Wiring Diagrams 2. Terminal Identification Diagrams 3. Point -to -Point Interconnection Diagrams 4. Description of Operation 5. Actual details of conduit installation B. Submit final As -Built Drawings and O&M Manuals. 1.03 FIELD ENGINEERING A. The Electrical Contractor shall determine the proper connection points for all power, control, and signal wiring, regardless of whether the connection points are in equipment furnished under this Contract, in equipment furnished by others, or in existing equipment. B. The Electrical Contractor shall perform all field surveys, wire tracing, and other work required to ascertain the proper connection points for all wiring. 1.04 INSUFFICIENT INSTRUCTION A. The Contractor shall furnish and install all materials and equipment which are obviously part of the complete electrical installation and without any additional charge to the Village of Barrington. B. If, in the opinion of the Contractor, any work shown on the Drawings or called for under these Specifications is insufficiently specified or specified in such a manner as to make it impossible for him to produce first-class work which will meet the approval of the Engineer, he shall refer same to the Engineer before proceeding with the work and, if he fails to refer such instances to the Engineer, no excuse for poor workmanship will be entertained. 1.05 CLASS I DIVISION I LOCATION INSTALLATION A. The Contractor shall furnish and install Crouse -Hinds Type EYS seals, or equal, wherever required according to Article 500 of the NEC. All wiring, conduit and appurtenances shall also conform to the NEC Standard for Class 1, Division 1 locations. The Contractor shall label each piece of equipment and assume responsibility for a safe reliable operating system. It will be the Contractor's responsibility to furnish and install conduit seals, explosion proof boxes, and equipment suited for Class 1, Division 1 location regardless of whether the equipment is specifically shown. PART 2 - PRODUCTS Not Used PART 3 - EXECUTION Not Used Page 13 Section E General Conditions 1. Contractor shall have five (5) years of experience which is comparable in type and scope to this project. 2. Work hours are 7:00 A.M. to 3:00 P.M. Monday through Friday. Any deviation is subject to the approval of the Village representative. 3. The project will be done on consecutive work days until completed, delays only to inclement weather. 4. Contractor is required to obtain all necessary permits from the Village of Oak Brook, and schedule required inspections through Community Development. S. The contractor shall supply to the Village, phone numbers where he/she can be reached after normal working hours. 6. The contractor must submit with the bid proposal five (5) references, names and phone numbers of similar projects completed within the last two (2) years. 7. The contractor must submit all manufacturers' literature on all materials that will be used on this project, including M.S.D.S. (Material Safety Data Sheets) prior to any work beginning. 8. A storage location for supplies, ladders and scaffolding shall be mutually agreed upon between the Village and the contractor before any material is stored on site. Deliver material with manufacturers labels intact and legible, store material on raised platforms and cover material with protective covering. 9. Before work is started, deliver to the job site sufficient material to complete the project. 10. If a dumpster is required, the location of the dumpster placement shall be mutually agreed upon between the Village and the contractor. 11. All ladders and scaffolding shall be maintained during the course of this project and shall be secured at the end of each workday. 12. Provide barricades to ensure that falling debris will not injure anyone, and to prevent public access to the work area at all times. Yellow "CAUTION" tape will be placed below the immediate work areas of laborers and scaffolds to warn the public of men working overhead. 13. At all times the work and storage areas shall be kept in a clean, orderly, and a picked up manner, to prevent debris from blowing. Clean adjoining streets and immediate vicinity at the end of each work day. Sidewalks, windowsills, roofs and other work areas will be broom swept to remove all debris. Daily material and debris not placed into dumpster will be removed from the site. 14. Upon completion of the project the work area shall be cleaned. All debris and remaining material and suppries shall be removed from the jobsite, including the dumpster within 72 hours of completion. 15. Upon completion the Contractor shall supply a one (1) year warranty covering material and workmanship; contractor shall submit sample warranty with bid.
