Insurance and Vision Insurance Agreements for 2014-15 Plan Year

RECOMMENDED MOTION: I move that the Village Board, consistent with the
recommendation of staff approve Resolution R-1376, "A resolution approving the waiver of
competitive bidding and authorizing the purchase of insurance benefits for employees."
Health Benefit Program:
Note: This year's changes are described in the Recommendation section.
The Village operates a self-insured health insurance program for all PPO claims on a per individual
basis for the first $85,000 ("individual specific attachment point") of claims in a given plan year
(July 1 to June 30). The Village also provides a fully insured HMO plan. This program has been in
place in this format since 1993, but with different specific attachment points and third party
administrators. The Village secures Stop Loss Coverage for claims that exceed $85,000 per
individual or over 125% of the annual aggregate attachment point of expected claims. The Village
health benefit program has approximately 164 employees on the PPO and 18 on the HMO, which
includes approximately 30 retirees (retirees/COBRA employees on the traditional insurance plan
who pay 100% of the equivalent premium for their coverage) in the program, plus eligible
dependents. The Oak Brook Park District is also on our plan paying the full premium cost for all
eligible individuals. The Village's program itself is a three-tier premium system consisting of
single, single +1 and family coverage for health, and two-tiers for dental and vision coverage with
network discounts through Blue Cross Blue Shield (BC/BS). In addition, the Village provides
group term life insurance coverage at$100,000 for all employees,with an option for only$50,000.
Renewal Information:
Our medical, dental, life and vision plans were all up for renewal. Our current consultants,
Mesirow Financial provided a complete analysis of our plans and costs. Their analysis began
with a review of the medical plan benchmarks on design, contributions, and costs. An analysis of
the impact of Healthcare Reform was also provided. In addition, Mesirow presented an analysis
of the claims utilization and provided recommended plan design alternatives for our review to
assist in lowering costs. In addition to negotiating with the current carriers Mesirow Financial
marketed all four of our plans. The complete analysis is available for review in the Village
Manager's office.
Medical: As for the BC/BS proposal, their initial overall renewal proposal was 3.9%
(approximately $113,000) higher than last year's rate. However, after negotiations and plan
adjustments, BC/BS's final proposal has a rate increase of 2.5% (approximately $77,500) higher
than our current plan year. When reviewing the BC/BS proposal, there are few key areas to
highlight. The first is the estimated fixed costs. This was increased by about 11% or
approximately $37,000. Next was claim liability, which remained the same. The Village has
experience a good year with respect to PPO claims being low. Finally, the HMO portion of the
medical is increasing about 10.9% or approximately $30,000. Lastly, there are the Affordable
Care Act (ACA) fees, which are decreasing January 1, 2015 on the PPO but increasing on the
HMO. The estimated total cost for the PPO will be around $23,000 for this plan year for the
transitional reinsurance fee. (This amount could have been drastically higher if the Village
wasn't self-funded, with amounts costing close to $85,500 for the 2014/2015 plan year.) The
medical reinsurance portion of the plan was marketed to six other providers who declined to
provide a quote. The medical rates are proposed for 1 year. The total estimated cost for the
medical plan is $2,679,000.
Dental: This year, the Village's dental insurance was also up for renewal. Delta Dental's total
renewal increase is approximately $1,100 or 0.7%. The dental plan was marketed to one
alternative provider and that proposal came back at a 2.25% increase over our current rates. The
dental rate is being proposed for 1 year. The total estimated cost for the dental plan is $159,000.
Life/AD&D: The life insurance and AD&D was also up for renewal. Mesirow marketed that
plan to 13 alterative providers and received five quotes back in addition to a proposal from our
current provider Dearborn National. The initial renewal offer from Dearborn National was an
increase of about $1,800 or 7%. Mesirow was able to negotiate the renewal increase to a no
change from the current rates. The total annual cost of the life insurance and AD&D benefit is
about $25,000 and they have agreed to a 2-year rate. The total estimated cost for the life and
AD&D plan is $25,000.
Vision: EyeMed is our current vision insurance provider and they proposed an increase of about
$600 or 2.99%, which is due to ACA taxes. They have proposed a contract period of 4 years
with their proposed rate for each of the four years. The total estimated cost for the vision plan is
$20,000.
The village will continue to promote its wellness program allowing those who receive insurance
up to 2% off their premium and for those without insurance the opportunity to receive gift cards
for achieving pre-determined wellness goals. Overall, the program is in transition this year and
has been scaled back due to a large reduction in our budget from $25,000 to $5,000.
Recommendation:
Based on the amount of substantial changes made to the insurance last year and in conjunction
with the favorable renewal rates, staff is recommending the following:
1. Effective January 1, 2015 for non-union emloyees, we recommend the PPO out of pocket
maximum be increased. For single coverage it will increase from $500 to $750 and for family
coverage it will increase from $1,500 to $2,250. The reason for this increase is that starting in
2015 prescription copays will be counted to towards out of pocket expenses, which means the
cost of the plan will be increasing. Also based on benchmarking data,the out of pocket
maximum combined with the deductible for our plans is low, approximately half of what is seen
in the private sector and other governemnt entities. It should be noted that the medical and
dental premium cost sharing percentages will remain the same as last year. Employees will
continue to pay fifty (50)percent of the Transitional Reinsurance Fee as described in the ACA.
2. Beyond the above adjustments,to continue to address the percentage of premium differential
gap in both the public and private sectors, staff is recommending maintaining the Tier 2 level of
contributions for full-time non-union employees hired after 1/1/2013 at a rate 5%higher than
employees hired before 1/1/2013 (Tier 1). The dental contributions are currently 20% for Police,
17.5% for Fire and 17.5/20% for non-union employees. Attached is a breakdown of the cost
sharing among employee groups.

Effective February 1, 2014,the Public Works union is no longer on the Village's health insurance plan.
They have joined their local union's health insurance group.

A RESOLUTION APPROVING THE WAIVER OF COMPETITIVE BIDDING AND AUTHORIZING
THE PURCHASE OF INSURANCE BENEFITS FOR EMPLOYEES
WHEREAS, the Village provides its employees insurance benefits (collectively, "Insurance
Plans"); and
WHEREAS, the Village currently purchases health insurance coverage, third-party administrator
services, and stop loss insurance from BlueCross BlueShield of Illinois (collectively, "BC/BS Plans"); and
WHEREAS, the Village currently purchases dental insurance coverage from Delta Dental ("Delta
Plan"); and
WHEREAS, the Village currently purchases vision insurance coverage from EyeMed ("EyeMed
Plan"); and
WHEREAS, the Village currently purchases term life insurance coverage from Dearborn National
("Dearborn Plan"); and
WHEREAS, the Insurance Plans are up for renewal; and
WHEREAS, instead of putting the purchase of the Insurance Plans out to bid or request for
proposals, the Village Board retained Mesirow Financial to negotiate with insurance providers to obtain
the most advantageous Insurance Plans for the Village; and
WHEREAS, because the Village retained Mesirow Financial to negotiate with insurance
providers, and the Village Board believes that Mesirow Financial has negotiated for the purchase of
Insurance Plans that are advantageous to the Village, the Village staff has recommended that the Village
waive competitive bidding for the purchase of the Insurance Plans; and
WHEREAS, Village staff has recommended the Village Board renew: (i) the BC/BS Plans so that
they continue through June 30, 2015; (ii) the Delta Plan so that it continues through June 30, 2015; (iii)
the EyeMed Plan so that it continues through June 30, 2018; and (iv) the Dearborn Plan so that it
continues through June 30, 2016 (collectively, "Renewals"); and
WHEREAS, the President and Board of Trustees, being fully advised in the premises, have
determined that it is in the best interests of the Village and its residents to so waive competitive bidding
and to approve the purchases of the Renewals all subject to the appropriation of sufficient funds for the
Renewals in future fiscal years;
NOW THEREFORE, BE IT RESOLVED BY THE PRESIDENT AND BOARD OF TRUSTEES OF
THE VILLAGE OF OAK BROOK, DU PAGE AND COOK COUNTIES, ILLINOIS as follows:
Section 1: Recitals. The foregoing recitals are hereby incorporated into, and made a part
of, this Resolution as the findings of the President and Board of Trustees of the Village of Oak Brook.
Section 2: Waiver of Competitive Bidding Requirements. The advertising and bidding
requirements for the purchase of the Insurance Plans are hereby waived in accordance with Section 1-7-
4 of the Village Code.
Section 3: Authorization of Renewals. The purchase of the Renewals is hereby authorized.

Section 4: Execution of Required Documentation. The President, the Village Manager, and
the Village Clerk shall be, and are hereby, authorized to execute and attest, on behalf of the Village all
documents necessary to complete the purchase of the Renewals pursuant to Section 3 of this Resolution.
Section 5: Effective Date. This Resolution shall be in full force and effect from and after its
passage by two-thirds of the Trustees and its approval in the manner provided by law.
