Attached is the '11991 Permit Resolution" for approval by the Village President and Board of Trustees. L Of • 201 West Center Court/Schaumburg,Nflnois/60196-1098 lose PERMITS eaY—r y resolution for Construction on State Aghw [ Mayor or Village President All Municipalities in District One Chapter 121 of the Illinois Revised Statutes requires that any person, firm or corporation desiring to do work on State-maintained rights of way must first obtain a written permit from the Illinois Department of Transportation. This includes any emergency work on broken watermains or sewers. A surety bond is required with each permit application to insure that all work is completed in accordance with State specifications and that the right of way is properly restored. 1 For permit work to be performed by employees of a municipality, a resolution is acceptable in lieu of the surety bond. This resolution does not relieve contractors hired by the -- municipality from conforming with the normal bonding requirements nor from obtaining permits. In order to expedite the issuance of permits to your . municipality during the calendar year, the attached sample resolution should be adopted and a signed and certified copy thereof returned to this office. This resolution does not constitute a blanket permit for work in the State system. A separate application must be made in each instance. In the case of an emergency, verbal authority may be given prior to receipt of the written application. After normal working hours or on weekends, this authorit can be obtained from our Communications Center at (708 705-4222. We would appreciate the cooperation of your community in withholding the issuance of building permits along State highways until the builder shows evidence of a State highway permit having been obtained. Our permit staff would be willing to answer any questions you may have regarding current policies or practices and to work with your planning commission on any new developments within your municipality. MEETING CALL: The Regular Meeting of the Board of Trustees was called to order by President Cerne in the Samuel E. Dean Board Room of the Village Commons at 7:32 p.m. ROLL CALL: PRESENT: President Wence F. Cerne Trustees Garry J. Bartecki Karen M. Bushy Ronald P. Maher Barbara A. Payovich Joseph H. Rush H. Richard Winters ABSENT: None PLEDGE TO THE FLAG II. APPROVAL OF MINUTES: Trustee Maher moved, seconded by Trustee Winters to approve the Minutes of the Committee-of-the-Whole Meeting of December 10, 1990 as presented and waive the full reading thereof. Trustee Maher moved, seconded by Trustee Winters to approve the Minutes of the Public Hearing of December 11, 1990 regarding the Comprehensive i Plan Ordinance as presented and waive the full reading thereof. Trustee Rush moved, seconded by Trustee Bushy to pass Resolution NA. R-515, the 1991 Permit Resolution, to allow Oak Srook to undertake work within the State right-of-way and waive the full reading thereof. ORDINANCE NO. 5-708 AN ORDINANCE REZONING PROPERTY TO CONSERVATION/RECREATION DISTRICT (1425 Kensington Road): The Park District, by letter of January 3, 1991, indicates that the Village should proceed with rezoning the property from the R-4 to the C/R District. It would be appropriate for the village Board to table the other two applications dealing with the text amendment and flood plain special use until such time as the Park District has prepared specific plans in this regard. Trustee Winters moved, seconded by Trustee Rush to pass Ordinance No. 5-708 as presented and waive the full reading thereof. Dome Corporation of America - Payout #1 & Final - Salt Storage Dome Construction (Public Works yard): Trustee Rush moved, seconded by Trustee Bushy to authorize payment of Payout #1 & Final to Dome Corporation of America for prefabrication and erection of L-50' Dia. Dome, 4' high foundation wall, roofing of same; including all labor, materials and equipment in the total amount of $49,740.00.
