RQLE5 +REDS , �-' Li FE zN SQRAA)CE 5-ZZ3'$9 ORDINANCE G- 429 AN ORDINANCE AMENDING ARTICLE IX OF THE PERSONNEL RULES & REGULATIONS OF THE VILLAGE OF OAK BROOK REGARDING LIFE INSURANCE BE IT ORDAINED by the President and Board of Trustees of the Village of Oak Brook, DuPage and Cook Counties, Illinois, as follows: Section 1: That Article IX, Section 1(C) of Personnel Rules & Regulations of the Village of Oak Brook be and is hereby amended to read as follows: "The Village provides life insurance for all full-time employees and assumes 100% of the cost of this benefit. Life insurance and accidental death and dismemberment insurance shall be provided to each such full-time employee during the term of their full-time employment in an amount equal to $50,000. Section 2: That this Ordinance shall be in full force and effect from and after passage and approval as provided by law. PASSED THIS 23rd day of May , 1989. The Village's present life insurance and AD & D policy is current salary rounded to the nearest $500 up to a maximum of $40,000 for all employees except Police and Fire Sworn personnel, who are limited to a flat $25,000. This policy costs the Village $15,553 annually. The three (3) options are listed below: 1. Life and AD & D of current salary rounded to nearest $500 for all Village employees, including Police and Fire Sworn personnel, to a maximum of $50,000. Cost to Village $17,604.60 annually. 2. Life and AD & D of current salary rounded to nearest $500 for all employees with no limit to maximum. Cost to Village: $17,729 annually. 3. Life and AD & D for all employees to a flat $50,000. Cost to Village: $28,860 annually. Recommendation: I have reviewed this matter with President Cerne, who suggests using Option #3. Option #2 is not recommended due to Internal Revenue Code and Social Security Act mandating that gross wages for employer provided group-term life insurance above $50,000 is to be taxed by FICA, Federal income taxes, and IMRF contributions. Option #1 is not recommended due to the potentially low amount of coverage that a lower-paid employee may receive. Option #3 is therefore recommended. It helps Police and Fire in that it eliminates their present limit of $25,000, as well as provides a uniform amount of insurance at reasonable costs. Option #3 also does not incur the FICA, Federal tax, and IMRF penalties that would result from option #2. Gallagher and A D & D Coverage 10 Gould Center Rolling Meadows, IL 60008 Dear Pam: The Village President and the Board of Trustees at their meeting of May 23, 1989, approved increasing the life insurance and the accidential death and dismemberment coverage to $50,000 per full time employee. Please instruct Washington National Life Insurance Company to make the adjustment necessary to accomodate this change. The increased coverage will be effective June 1, 1989, and the Village's June billing will reflect the new coverage limits.
