Personnel Manual Amendments

RECOMMENDED MOTION: I move to pass Ordinance 2004- PL -RR -S- amending the
text of the Village of Oak Brook Personnel Manual.
Background /History: Pursuant to your direction, I have prepared an ordinance amending the
Village of Oak Brook Personnel Manual raising the employee
contribution to health and dental insurance, replacing the words "fire
captains" with "battalion chief', and raising the amount of accrued sick
leave that can be paid upon separation following ten (10) years of
continuous full -time service.
Recommendation: I recommend that this Ordinance be passed.

Resolution 2004- FLSA -R -875, "A Resolution Amending the Fair Labor
Standards Act Policies for the Village of Oak Brook," conforming to
current law and altering certain titles of positions ROLL CALL VOTE:
Ayes 5 - Trustees Aktipis, Caleel, Korn, Miologos and Yusuf.
Nays: 0 - None.
Absent: 1 - Trustee Craig. Motion carried.

1062- AMENDS
ARTICLE V OF THE PERSONNEL MANUAL OF THE VILLAGE ART V- PERSONEL
OF OAK BROOK, ILLINOIS" MANUAL
The ordinance amends the Village of Oak Brook Personnel Manual
raising the employee contribution to health and dental insurance,
replacing the words "fire captains" with "battalion chief', and raising
the amount of accrued sick leave that can be paid upon separation
following ten (10) years of continuous full -time service.
Motion by Trustee Konn, seconded by Trustee Caleel, for passage of
Ordinance 2004- PL -RR -S -1062, "An Ordinance Amending Article V of
the Personnel Manual of the Village of Oak Brook, Illinois," raising the
employee contribution to health and dental insurance, replacing "fire
captains" with "battalion chief' and raising the amount of accrued sick
leave allowed. ROLL CALL VOTE:
Ayes: 5 - Trustees Aktipis, Caleel, Konn, Miologos and Yusuf.
Nays: 0 - None.
Absent: 1 - Trustee Craig. Motion carried.

HEALTH AND DENTAL INSURANCE HEALTH & DENTAL
The Village of Oak Brook is self - insured for most employee benefit
programs. The Village utilizes the services of Mack & Parker, Inc. (on a
monthly retainer basis) for plan administration and benefit consulting
services. Incentus Benefits is the current third party administrator for
claims processing. The plan renewal normally takes place on July 1
each year with both Mack & Parker and the third party administrator
assisting the Village in obtaining quotes for the various stop -loss
insurance coverage, PPO access charges, etc.

ORDINANCE 2004- PL -RR -S -1062
AN ORDINANCE AMENDING ARTICLE V
OF THE PERSONNEL MANUAL OF THE VILLAGE OF OAK BROOK, ILLINOIS
WHEREAS, the President and Board of Trustees of the Village of Oak Brook have heretofore adopted the
Village of Oak Brook Personnel Manual, and
WHEREAS, it 1s deemed appropriate that said Personnel Manual be amended;
NOW, THEREFORE, BE IT ORDAINED BY THE PRESIDENT AND BOARD OF TRUSTEES OF
THE VILLAGE OF OAK BROOK, DU PAGE AND COOK COUNTIES, ILLINOIS as follows
Section 1 That the words "fire captains" be deleted any place that they appear in the manual and that the
words "battalion chiefs" be inserted in their place.
Section 2. That Article V, Section 5 02 D 1 of the Personnel Manual of the Village of Oak Brook be and is
hereby amended to read as follows:
25% of accrued sick leave, not to exceed 180 days, or the equivalent of 2,016 hours for the
battalion chief, at the employees' then rate of pay, or
Section 3 That Article V, Section 5.12 A of the Personnel Manual of the Village of Oak Brook be and is
hereby amended to read as follows
A Health, Dental & Vision
The Village of Oak Brook is self - insured, and offers medical and dental benefits under a group
health plan Currently the options of an Indemnity plan or a Preferred Provider Organization
(P P O ) plan are available The Village also offers benefits through a Group Vision Care Plan
Full -tune employees and their eligible dependents may participate. Regular part-tune employees
normally scheduled to work at least 20 hours per week may also elect to participate by paying the
full equivalent premium cost of the selected coverage (i e employee, employee plus one or
family) Coverage commences as follows-
Medical on the 91 st day of employment
Dental on the 181 st day of employment
Vision on the 1 st day of the month following the date of employment
The Village also offers an Internal Revenue Code Section 125 flexible benefit plan whereby
employees may contribute fiends, on a pre -tax basis, to pay out -of- pocket medical, dental, vision
and child care /dependent care expenses
The plans are fully described in the Summary Plan Descriptions which are attached as Appendix C
(Health and Dental Plan), Appendix D (IRC Section 125 Plan) and Appendix E (Vision Plan)
They may be amended from time to time in accordance with their terms and applicable law
Eligible employees who elect health and dental insurance shall be obligated to pay 7 -1/2% of the
premium cost of the selected coverage (i e employee, employee plus one or family)
Section 4 That this ordinance shall be in full force and effect from and after its passage, approval and
publication as required by law

same are hereby repealed to the extent of such conflict
