RECOMMENDED MOTION
I move that the Village Board approve revising the current Fire Departmental Fee
structures as outlined under Village Ordinance Section 3-2-5, Paragraph A, Emergency
Medical Services and amend the user charges for levels of service and impose a standard
professional services fee to residents and non-residents who receive emergency medical
services treatment with transportation.
Background/History:
Since 1994, the Fire Department’s Emergency Medical Services has been, in the course of
providing such service, deemed it appropriate to impose a user charge for such service in order to
partially defray the cost to the Village for providing such service, a fee to any patient receiving
emergency medical services resulting in transportation to an emergency department.
The current fee structure, revised and adopted in 2011 reflects the following;
1. Five hundred fifty dollars ($550.00) for residents of the village plus ten dollars ($10.00) per
mile from the point of pick up to the hospital for basic life support services.
2. Seven hundred dollars ($700.00) for residents of the village plus ten dollars ($10.00) per mile
from the point of pick up to the hospital for advanced life support level I services.
3. Eight hundred dollars ($800.00) for residents of the village plus ten dollars ($10.00) per mile
from the point of pick up to the hospital for advanced life support level II services.
4. Six hundred fifty dollars ($650.00) for nonresidents plus ten dollars ($10.00) per mile from the
point of pick up to the hospital for basic life support services.
5. Eight hundred dollars ($800.00) for nonresidents plus ten dollars ($10.00) per mile from the
point of pick up to the hospital for advanced life support level I services.

6. Nine hundred dollars ($900.00) for nonresidents plus ten dollars ($10.00) per mile from the
point of pick up to the hospital for advanced life support level II services.
Basic life support services are defined as those services which are not life threatening and do not
require the intervention of advanced life support treatment including, but not limited to the
establishment of an intravenous line, cardiac defibrillation and the administration of system
approved medications.
Advanced Level 1 services are those services not categorized as basic life support and which
require the intervention and treatment, which includes, but is not limited to the establishment of an
intravenous line and the administration of less than three (3) system approved medications.
Advanced Level 2 services are those services not categorized as basic life support and which
require the intervention and treatment, which includes, but is not limited to the establishment of an
intravenous line, cardiac defibrillation, CPR, endotracheal intubation and the administration of
three (3) or more system approved medications.
A detailed survey of current emergency medical service fees of departments and organizations
throughout much of our metropolitan area have shown an average of the following fees;

Our internal audit of fees collected in 2017 reveal the following statistics;

The percentages of our collections under Medicare and private insurance are the largest revenue
source. Medicare eligible patients, regardless of the type of service, fee or charges pays a flat
$435.68 per call. General coverage under a large number of private insurance carriers cover 80%
of emergency ambulance services. Privately insured patients are ultimately responsible for the
remaining balance following assignment through their insurance carrier.
Following our recent audit and after a careful and thorough analysis of current industry trends,
we have determined that amending the current fee structure to a standard single
PROFESSIONAL SERVICES fee would simplify our billing process, improve our percentage of
collections and greatly increase the revenue generated by such a fee.
Similar to our current fee structure, a professional services fee would be established differently
for residents and non-residents. Additionally, our current charge imposed per mile from the
point of pick up to the hospital should also reflect an increase reflecting an average within the
market place.
The following chart represents the new proposed professional fee structure for consideration and
implementation;

To provide a perspective of the going rates within the market, I have attached a lengthy
spreadsheet compiled by the Naperville Fire Department. It is a very thorough survey
representing a wide range of agencies across the surrounding area. Other communities which
have recently increased their fee structure using the modified professional services fee include,
but are not limited to Addison, Itasca, Lisle-Woodridge, Lombard, Naperville, Plainfield and
Wood Dale.
Taking into account the actual call volume, types of calls and fees generated and collected for
2017, and using that data inserting the proposed professional services fee structure, an additional
$250,000.00 dollars would have been generated.
Recommendation:
That the above motion be presented to the board for approval.

AN ORDINANCE AMENDING SECTION 3-2-5 OF TITLE 3
OF THE VILLAGE CODE OF THE VILLAGE OF OAK BROOK
RELATIVE TO FEES FOR USE OF EMERGENCY MEDICAL SERVICES
PROVIDED BY THE OAK BROOK FIRE DEPARTMENT
WHEREAS, the Village President and Board of Trustees of the Village of Oak Brook have
heretofore passed and approved Ordinance G-948 on September 13, 2011, which revised the fees
charged to any person receiving emergency medical service and transportation provided by the Village of
Oak Brook; and
WHEREAS, the residents of the Village pay certain taxes, the revenue from which support the
cost of emergency medical service and transportation; and
WHEREAS, the President and Board of Trustees of the Village of Oak Brook have reviewed the
emergency medical fees and determined that adjustments in the fees are necessary to more properly
reimburse the Village for the costs of providing the services.
NOW, THEREFORE, BE IT ORDAINED BY THE PRESIDENT AND BOARD OF TRUSTEES OF
THE VILLAGE OF OAK BROOK, DU PAGE AND COOK COUNTIES, ILLINOIS as follows:
Section 1: The recitals are adopted as though fully set forth in this Section.
Section 2: Section 3-2-5 of the Village Code of the Village of Oak Brook be and is hereby
amended to read as follows:
3-2-5: DEPARTMENTAL FEES:
A.Emergency Medical Services: As the Village, through its fire department, currently
provides emergency medical service to the Village, and the corporate authorities of the
Village deem it appropriate to impose a user charge for such service in order to defray
partially the cost to the Village of providing such service, a fee is hereby imposed upon
any person receiving emergency medical service and transportation for each use
provided by the Village as follows:
1.Five Sixteen hundred fifty dollars ($550.00 1,600) for residents of the Village
plus ten nineteen dollars ($10.00 19.00) per mile from the point of pick up to
the hospital for basic life support services.
2.Seven Sixteen hundred dollars ($700.00 1,600) for residents of the Village
plus ten nineteen dollars ($10.00 19.00) per mile from the point of pick up to
the hospital for advanced life support level I services.
3.Eight Sixteen hundred dollars ($800.00 1,600) for residents of the Village
plus ten nineteen dollars ($10.00 19.00) per mile from the point of pick up to
the hospital for advanced life support level II services.
4.Six Eighteen hundred, fifty dollars ($650.00 1,800.00) for non-residents plus
ten nineteen dollars ($10.00 19.00) per mile from the point of pick up to the
hospital for basic life support services.
5.Eighteen hundred dollars ($1,800.00) for non-residents plus ten nineteen
dollars ($10.00 nineteen) per mile from the point of pick up to the hospital for
advanced life support level I services.

6.Nine Eighteen hundred dollars ($900.00 1,800.00) for non-residents plus ten
nineteen dollars ($10.00 19.00) per mile from the point of pick up to the
hospital for advanced life support level II services.
B. Extrication Fee: In addition to the fees required pursuant to subsections A and C
of this section, when members of the fire department are required to use
hydraulic, gas powered, electric powered, air powered, or any other specialized
rescue equipment to remove a resident or nonresident from a vehicle or other
structure, such person shall pay the village an additional service fee of three
hundred dollars ($300.00).
C.Cardiopulmonary Resuscitation And First Aid Classes: The village, through its
fire department, currently provides cardiopulmonary resuscitation and first aid
instruction to interested persons, and the corporate authorities of the village
deem it appropriate to impose a user charge for such service in order to defray
partially the cost to the village to provide such service. Fees, as determined by
the village board or such other fee established by the village manager upon
consultation with the village board are imposed per the table below:

Section 2: That this ordinance shall be in full force and effect from and after its passage,
approval and publication as required by law.
Section 3: That all ordinances or parts thereof in conflict with the provisions of this ordinance are
hereby repealed to the extent of such conflict.
