The following is a summary of changes to be made to the Construction Manager's Bid Manual for this project. Please review and incorporate all changes into your bid. This addendum shall be noted on the bid form verifying inclusion of this work in your bid. ADDENDUM NO. 1 1. The following change applies to all bid packages: A. All addenda shall be noted on the front page of the bid form. Bid addenda can be verified on PROCORE. 2. In the SCOPE OF WORK sections, make all changes as outlined below: A. Bid Package #03 – Cast-in-Place Concrete – Replace with new section attached B. Bid Package #04 – Masonry – Replace with new section attached. C. Bid Package #06A – General Trades – Replace with new section attached D. Bid Package #06B – Detention Equipment Work – Add this new scope of work section to the project. E. Additional testing for failure of work will be at the Trade Contractors expense. ( General Testing includes: Soil, Concrete, Paving, and Steel Testing only ) Refer to plans and spec books for testing as required. Contractor is required to clean their work area on a daily basis and dispose of any/all material not required for the execution of their work. The Construction Manager will provide clean-up on a daily basis for all contractors not cleaning after themselves, and will back charge the contractor for costs of labor and material plus 15% for these services. Locate all existing underground utilities and maintain a current dig # during entire time of site work. Provide all MSDS, for products used on this project. This contractor is responsible for protecting property and improvements. This contractor will restore any grass or landscaped areas, disturbed during their phase of work. There will not be temporary power available during this work. This contractor shall provide his own temp power. All foundations are to be formed - no earth form, unless approved by engineer in writing. Include rebar, wire mesh for curbs, walks and drives (barrier curb, depressed curb) as per plans. General All work by this contractor is to meet and/or exceed all of the State of Illinois, County & local building codes. Construction Manager will provide a benchmark, building corners, parking lot corners, and center of road will be located, additional layout by contractor. Contractor retainage will not be released until completion of Contract Documents, Project Record Documents, Operating and Maintenance Data, and Warranties and Bonds are submitted for acceptance by Engineering and Architectural firms. Contractor will be responsible for keeping the Roads, City Streets, State Highway clean of mud and debris. This contractor will repair, replace any silt fence damaged or removed during his work Concrete Testing provided by Construction Manager. Submit shop drawings and cut sheets within 2 week of Construction Manager's - 'Notice to Proceed' / 'Letter of Intent'. Where the combination of the shop drawing preparation/review and fabrication time frame exceeds the time allowed in the milestone schedule, this contractor shall notify the Construction Manager immediately upon issuance of the 'Notice to Proceed' / 'Letter of Intent'. Include but not limited to the following: Scope of Work Notes, Project Specifications, Construction Documents Furnish and install all site concrete work as shown and specified including but not limited to; concrete at sidewalks, curb & gutter, depressed curbs, concrete pavement, aprons, etc. All granular fill below site concrete work will be furnished and installed by this Contractor. Furnish all required wash-out equipment as required to collect and contain concrete washout debris for your work. Concrete washout debris will be removed from the site as soon as possible by this contractor but not later than the conclusion of work. Debris will not allowed in Construction Manager dumpster, nor will contractor be allowed to discharge wash-out into/onto the ground. Provide all labor, material and equipment as required to replace concrete curbs, walks, etc. at areas removed for site utility work. Include finishes on sidewalks, handicap ramps, pads, etc. as per contract documents. Include hand and machine excavation as required to achieve correct bearing elevation. Include Handicap ramps, detectable warning surfaces and special finishes as per contract documents. Replace upper sidewalk along parking lot, matching existing concrete profile/finish, as indicated on the site logistics plan. Provide all stone subbase as required. The Cast-In-Place Concrete Package shall include, but not be limited to, all materials, labor, equipment, samples, shop drawings, engineering, layout, supplies, plant, tools, contributions, insurance, taxes, compliance with all agencies (City, County, State, and Federal as may be required) all other services and facilities, and other things necessary for the performance of the work as shown, detailed, and/or implied by the Contract Documents as listed in the invitation to bid and as defined herein. Include foundation insulation at slab edge, perimeter wall insulation, etc. as shown and indicated on the contract documents. Cast-in-Place Concrete Construction Manager will provide layout indicated in the Bid Package 2A - Surveying. Any additional layout required for this work will be by this package. Cast-in-place concrete as indicated in the Contract Documents. Work shall include but not be limited to slab-on- grade areas including thickenings, toppings, depressions, foundation footings and walls, perimeter frost wall, slab-on-grade (at new addition and interior renovation) including vapor barrier, slab-on-grade areas at entries (coordinate slab recesses with other trades for walk-off mats, bar gratings, etc.), detention bunk and benches (handcuff rings provided by Bid Package #6B and installed by this package). Included are all hoisting, pumping and any layout associated with the installation of this work. Provide exterior frost walls at all entrances and exits for concrete slabs to bear on. Include foundations and stoops and approaches at exterior door locations as per plans and specs. Include floor slope as shown and indicated for slab-on-grades. Unload, distribute, layout and install all embedded steel items in cast-in-place concrete. It is the responsibility of the Concrete Contractor to coordinate the locations of anchor bolts and embedded steel items with respective contractors. The Masonry and Structural Steel Contractors will provide Concrete Contractor with engineered and approved anchor bolt and embedded steel layout drawings. Tolerances of templates and embedded steel to be +\- 1/8” in any direction. Construction Manager to be notified within 24 hours of any discrepancies between drawings. Include all expansion joints, control joints, construction joints, as required by manufacturer, project specs. Control joints to be installed within 4 to 12 hours after concrete placement. Any concrete slabs at exterior door or window locations are to terminate on exterior side of door, so that there are not any joints under the door. Include placement of slab on grade, door stoops, etc. as shown and indicated Furnish and install all formed box outs, sleeves, etc., necessary for horizontal penetrations into all vertical cast-in- place concrete surfaces. Furnish and install all sleeving, bulkheads, box outs, etc., required in horizontal surfaces where vertical penetrations exist or occur. Coordinate with other trades requiring the openings. Floor flatness shall be as specified in contract documents. This contractor shall verify floor flatness using accepted industry practices. This package shall be responsible for any additional concrete above minimum indicated on the contract documents to meet contract levelness requirements. Furnish and install vapor barrier material, as shown and specified. This contractor shall utilize water reducing admixtures as allowed per the contract documents to reduce water/cement ratios in new concrete slabs to help in achieving acceptable Rh moistures for flooring. In addition this contractor shall use due caution in the finishing of the slabs to prevent over trowelling of the slab and the sealing of pores which could prevent the evaporation of free water from the slab. This Contractor is to meet at the site with the Site Utilities Contractor to review final elevations of all catch basins included in the areas of this work. Adjustment to final elevations will be by the Site Utilities package. Include caulking, sealers, expansion joints, mesh and rebar as required in and adjacent to site concrete work installed by this package as shown and indicated on the contract documents and as required to provide a complete job.. End of Bid Package #3 - Cast in Place Concrete Coordinate with Flooring Contractor and Plumbing Contractor for proper elevations at floor drains and floor cleanouts. Review Milestone Schedule for project phasing requirements. Provide equipment and manpower (multiple crews) as necessary to meet the Milestone Schedule dates. This Contractor understands that all work within this package may not occur at one time. Multiple mobilizations are assumed and contained in this package as required. Provide an anchor bolt bearing plate survey upon completion of this work, prior to the installation of the structural columns and posts, for review by the Structural Steel Contractor. Allow sufficient time to allow Structural Steel Contractor to make minor adjustment in steel for process. The Concrete Contractor and Structural Steel Contractor must meet and agree where rework of anchor bolts and/or revision to base or setting plates. This issue will be settled directly between the contractors. The work shall not be delayed by claims for refusal to commence corrections. Include pumping of surface water run off into foundation trench. Provide hand grading for areas of thickened slab. This package shall be responsible for furnishing and installing any required concrete curing methods or compounds including wet cure where required. All curing methods shall be compatible with specific floor finishes. Furnish all required wash-out equipment as required to collect and contain concrete washout water / debris for your work. On-Site wash out will be limited to cleaning of chutes only. Complete truck wash-out shall occur off-site. Concrete washout debris will be removed from the site. Debris will not be allowed in Construction Manager dumpster. All construction waste removed from the site shall be recycled in accordance with project construction waste recycling program. This contractor shall provide documentation on a monthly basis verifying percent of waste recycled from this project. Twenty-four (24) hour notice will be the minimum standard in notifying Construction Manager and Testing Agency of a scheduled pour. This Contractor is responsible for scheduling all testing required in this bid package. The Construction Manager will not schedule testing. Winter weather protection and hot weather concrete operations shall include insulated blankets, enclosures and/or temporary heat and any other means required by the atmospheric temperature tolerances of the specifications to ensure a continuous operation. Protect slabs, footings, foundation walls and exposed excavations. Prior to the installation of any flooring materials, the Concrete Contractor and the Flooring Contractor must meet and agree on the locations where latexing and leveling is needed. This is to determine that the quality of the concrete finish is acceptable to the Flooring Contractor and in accordance with the specification. Cost of repairs to new slabs will be by the Concrete Contractor. However, minor latexing and leveling will be the responsibility of the Flooring Contractor. The lack of an agreement shall not delay the work. Structural members may not be cut, notched, or altered in any way, without the consent of Construction Manager, Architect, and Engineers prior to alteration. Include all mechanical pads for equipment as per plans and specs. Grout in rails after equipment is set. Include cost of placement, either pumping or conveyor system in your proposal. Include concrete topping, stair pan fill, etc. Install steel bollards, including installation, concrete, etc., at all locations shown and indicated on the contract documents and at outside corners of all overhead sectional garage doors. Masonry 1 The Masonry Package shall include, but not be limited to, all materials, labor, equipment, samples, shop drawings, engineering, layout, supplies, plant, tools, contributions, insurance, taxes, compliance with all agencies (City, County, State, and Federal as may be required) all other services and facilities, and other things necessary for the performance of the work as shown, detailed, and/or implied by the Contract Documents as listed in the invitation to bid and as defined herein. 2 All scaffolding and vertical hoisting for the masonry work. This Contractor is responsible for the engineering, erection, maintenance and removal of all scaffold as required to meet OSHA regulations. Attachment method to building is by this Contractor. Do not overload structure. 3 All specified mockups, including removal and placement into dumpster. Provide additional support and concrete base if required for mockup. Remove mockup as directed by the Construction Manager. 4 All flexible fillers as required where your work meets new materials (i.e. flooring above, metal deck, masonry etc.). Include fire safing insulation between the tops of non load-bearing masonry walls and structural deck members. 5 Install all lintels and miscellaneous steel embedded in masonry work. Lintels and miscellaneous steel will be furnished by the Structural Steel Contractor. Masonry Contractor will coordinate quantities and lengths with the Structural Steel Contractor prior to delivery of lintels. Mason assumes responsibilities for unloading/storage of lintels. Masonry Contractor and Structural Steel Contractor are to meet to coordinate delivery schedule of all items for installation by Masonry Contractor to meet dates indicated on milestone schedule. This contractor is responsible to weld all lintels to bearing plate/setting plate connections of materials that have been installed by this contractor. General Shales McNutt Construction Scope of Work Notes Village of Oak Brook Police Detention Zone Renovation Addition Dated: September 19, 2017 Add #1: September 28, 2017 Masonry Bid Package # 4 Include but not limited to the following: Scope of Work Notes, Project Specifications, Construction Documents 6 Furnish and install reinforcing, anchors, ties and any masonry accessories required for the work (Welding of anchors to steel enclosed in masonry will be by Structural Steel package. Masonry Contractor is to furnish these anchors to Structural Steel Contractor for shop attachment to the steel). Masonry Contractor and Structural Steel Contractor are to meet to coordinate delivery schedule of all items for installation by Structural Steel Contractor to meet dates indicated on milestone schedule. 7 Mason to provide openings for HVAC openings, etc. Coordinate with HVAC Contractor for size and locations. 8 All cutting of block or brick, all special brick shapes, architectural stone, bands and sills. All cutting shall meet OSHA requirements. 9 Coordinate all work with security detention equipment as required. Install handcuff rings, etc. as furnished by Bid Package #06B as required 10 Furnish and install fluid-applied membrane air barrier system at masonry cavity walls where shown and indicated on the contract documents. 11 All masonry walls are in this package. Foundations and footings are by the Cast-in-Place contractor. 12 Install all required access panels in masonry walls as furnished by the MEP contractors. Layout is by the MEP contractors. 13 Coordinate the installation of all fire hose/extinguisher cabinets, hose bibs, back boxes, etc., with the necessary contractors. 14 Layout for your work. Coordinate and assist with layout of thickened slab for masonry walls with Concrete Contractor. 15 Provide all flashings indicated for masonry work. 16 Mixing area location will be coordinated with Construction Manager. 17 Provide bond beams with reinforcing and grout solid. 18 Coordinate installation of Electrical and HVAC equipment installation with Electrical and HVAC Contractors. 19 Cleaning of masonry walls. Leave window jamb returns clean from mortar to allow installation of windows. Coordinate cleaning chemical and procedures with the Glass and Glazing Contractor to avoid damaging the glass and/or frame finish. Multiple mobilizations may be required for masonry cleaning; coordinate with Milestone schedule and Window installation. 20 Include caulking of interior and exterior expansion and controls joints, corners, etc. for all masonry work. Where specified security caulking is required, that work will be performed by bid package #06A. 21 Coordinate with Concrete Contractor for any items to be cast in concrete. 22 Coordinate with MEP contractors for any sleeves as required. Sleeves and layout to be provided by the respective MEP contractors prior to wall erection, and installed by this package. 23 All remedial work required due to bad test results, incorrect layout/elevations, fabrications, will be the responsibility of the Masonry Contractor including costs incurred for re-testing. 24 Protect open cavity walls. Protection of partially completed cavity walls is the responsibility of the Masonry Contractor. This includes open cavities awaiting completion of roofing activities. 25 Review milestone schedule for project phasing requirements. Provide equipment and manpower (multiple crews) as necessary to meet the milestone schedule dates. 26 This Contractor understands that all work within this package may not occur at one time. Multiple mobilizations are assumed and contained in this package as required. 27 Leave openings for steel installation at connections for bolting/welding and inspection. Fill openings immediately after connections are complete and inspected. Coordinate with the Steel Contractor. 28 Brace all new walls. Bracing to be consistent with the current edition of the Masonry Council of America’s ‘Standard Practice for Bracing Masonry Walls Under Construction’. Remove at completion of steel framing/decking systems or direction of Construction Manager. 29 Roofing Contractor to provide any embedded items to Masonry package for installation. Roofing Contractor will take responsibility for items not delivered to Masonry Contractor in time to meet all milestone dates. Roofing and Masonry Contractors are to meet to coordinate schedules. 30 Mason is to meet with the Painting Contractor before and after the installation of the painter’s block fill application to determine areas of masonry requiring touch-up/repair. 31 All cells at top course of block walls are to be filled solid where required by plans. Carpentry 1 All wood blocking, including but not limited to roof edge, metal panel, access ladder, aluminum window and glazing, door, complete with anchors, for casework, toilet accessories, windows, etc. is included in this package. However, minor window shimming is included in the window package. Furnish anchors for blocking to masonry contractor for installation, as required. Coordinate window blocking requirements with Bid Package #8 Glass and Glazing 2 Wood blocking not shown on the Construction Documents but required for the completion of the work is the responsibility of the Contractor requiring the work. 3 Furnish and install all wood sheathing as per contract documents. Gypsm based sheathing will be installed by Bid Package #9A - Drywall & Acoustic. 4 Furnish and install blocking/backing for handrails, toilet partitions and accessories, all cabinetry and countertops, etc., at heights specified to meet all ADA requirements. As required by plans and specs. 5 Structural members may not be cut, notched, or altered without the consent of Construction Manager, Architect, and Engineers prior to alteration. 6 Door rough openings are not to exceed ¼” tolerance. All door openings are to be plumb, square and follow manufactures specs. 7 Coordinate delivery of products, supplies with Construction Manager Superintendent. 8 Install hollow metal doors, frames and hardware as per plans and specs including grouting of frames where specified. 9 This contractor will provide to the CM a copy of manufacturer installation instructions, supplied with each product. 10 Coordinate delivery of products, supplies with Construction Manager Superintendent. Provide equipment and manpower (multiple crews) as necessary to meet the milestone schedule dates. 12 This Contractor understands that all work within this package may not occur at one time. Multiple mobilizations are assumed and contained in this package as required. 13 Furnish and install plywood backboards for electrical, temperature controls and low voltage packages. 14 Include the value of $5,000.00 in bid. The contractor will be instructed as to how these funds are applied. Unused funds will be deducted from contract after approval by Owner and Construction Manager thru the Change Order Process. Drywall 1 Interior drywall partitions, soffits, light shelves, thermal and sound batt insulation, shaft wall studs, furring channels, light gage framing, drywall ceiling systems, taping of drywall work, reglets and reveals at drywall partitions. 2 This contractor shall provide all labor, material and equipment to vacuum all tracks prior to installation of gypsum board. 3 Include all expansion joints, control joints, construction joints, as required by manufacturer, project specs. 4 All exterior metal stud framing for canopies, fascias, and soffits. Provide engineered design as required by contract documents. 5 Furnish and install all exterior sheathing as shown and indicated where shown and indicated. 6 Include caulking at the junction of drywall and dissimilar materials, at the tops of drywall partitions and the structure above and at control/expansion joints in drywall, where indicated in the documents. 7 Furnish and install access panels specifically shown on the drawings. Access panels not shown on the drawings, but required by the MEP contractors, and located in drywall ceilings and/or walls, shall be furnished by the MEP contractors and installed by the Drywall contractor. Layout is by the MEP contractors. Access panels, shown on drawings, but located in masonry, will be provided by this package and installed by the masonry contractor. All access panels shall be keyed alike. Coordinate with CM for keying requirements, and tag and bag key for turn over to CM. Coordinate all work with cast-in-place concrete, masonry, general trades and MEP contractors for a complete installation. 2 Furnish and install all detention grade (pick-proof) caulking in detention areas in walls, floors and ceilings as required. During excavation in interior spaces, use appropriate cautions to prevent the emission and build-up of carbon monoxide gas. Excess excavation spoils from underground work generated by this contractor shall be removed from the site and legally disposed of by this bid package. Any required testing for proper disposal compliance is the responsibility of this bid package. 10 All layout as required for work included this package. 11 This contrator shall review power requirements for all equipment submittals of other trades on the project for verification of electrical requirements. Discrepancies shall be brought to the attention of SMC and the design team immediately. This contractor will be responsible for any and all requirements to furnish and install power to provided equipment at no additional cost to the project if he fails to bring attention to discrepancies in a timely manner. 12 Temporary electrical lighting and power for project in accordance with OSHA requirements. Remove all temporary installations when no longer needed. Requirements consist of the following: a. Temporary Lighting: All areas of the building shall be illuminated and wired per OSHA requirements. b. GFCI Power Outlets: Provide a minimum of six (6) GFCI protected temporary outlets and temporary 110 amp service at locations in each building such that maximum extension chord length does not exceed 100 ft. Energy consumption will be paid by the CM/Owner. 13 Furnish only all access panels required for your work. All access panels shall be keyed alike. Coordinate with CM for keying requirements, and tag and bag key for turn over to CM. Raceway, when not able to be run in walls, shall be run at right angles and/or parallel to floors and ceilings. 22 All equipment pads shown on the drawings will be by the Concrete Contractor. Any additional concrete pads that might be needed and are not shown on the drawings are the responsibility of this bid package.
