Chapter 121 of the Illinois Revised Statutes requires that any person, •
firm or corporation desiring to do work on State-maintained Rights of
• May must first obtain a written permit from the Illinois Department • •
of Tr'„eportation. This includes any emergent,/ work on broken water*-
mains or sewers.
A surety bond is required with each permit application to ,insure that
all work is completed in accordance with State Specifications and that
. the right of way is properly restored.
For permit work to be performed by employees of a municipality, a
resolution is acceptable in lieu of the surety bond. This resolution
would,not relieve contractors hired by the municipality from conform-
ing with the normal bonding requirements.
In order to expedite the issuance of permits to your municipality during.
the calendar year the attached sample resolution should be adopted
and a signed and certified copy thereereturned to this office. This
resolution does not constitute a blanket permit for work in thoeState
system. A. separate applications west be made in each instance.e. In.
the case of an emergency, verbal authority may be given prior to
receipt of the written application. After normal working hours or on •

SUBJECT: 1981 Yearly Resolution for Construction on State Highways
Attached please find the 1981 Permit Resolution which the Village of Oak Brook If
should execute and forward to the State of Illinois. Also attached is a copy
of a letter meted February 1, 1980 from James M. Graziano, District Peirmi
Engineer, Illinois Department of Transportation which explains the need for j
this Resolution. 1. have recently spoken with fir. Grasieno- regarding the 1991
Permit gesolution, with his conclusion being that a 1981,,Resolgmlon similar
to tithe 1980 Resolution would be satisfactory to IDOT. Therefore, i am also 'i .
attaching a copy of Resolution fit..267 "1980 Permit Resolution".
This is being placed on the Board Agenda. of December 16, 1980 for your
consideration.

To accept the proposal form International Surplus Lines Insurance Company
in the amount of $4,048.00.

To approve final payout for Oak. Brook Golf Course Clubhouse in the amount
of $27,730.00 to Mangan Builders, Inc., which includes the deduction for
concrete removal & curb repair, subject to receipt of
1) As-builts of the entire building certified by the contractors.
2) Guarantee from Mangan, Inc.
3) Approval of final waivers.
